FIG. 1 is a perspective view of the front and top sides the booklet shown in a closed position;

FIG. 2 is a front view thereof;

FIG. 3 is a left side view thereof;

FIG. 4 is a rear view thereof;

FIG. 5 is right side thereof;

FIG. 6 is a top plan view thereof, the bottom plan view being a mirror image;

FIG. 7 is a perspective view of the front and top sides of the booklet shown with the booklet cover folded open in a position of use;

FIG. 8 is a front view of FIG. 7;

FIG. 9 is a left side view of FIG. 7;

FIG. 10 is a rear view of FIG. 7;

FIG. 11 is a right side view of FIG. 7;

FIG. 12 is a top plan view of FIG. 7 the bottom plan view being a mirror image;

FIG. 13 is a perspective view of the front and bottom sides thereof with the booklet cover in a partially open configuration to clearly show the inner sides of the panels forming the cover; and,

FIG. 14 is a perspective view of the rear and bottom sides of FIG. 13.
