    Abstract: Embodiments of the invention describe methods of implanting and setting an adjustable suture structure capable of maximizing and controlling the available range of motion in a defective joint. The adjustable suture structure can augment a joint repair following surgery or be used as a standalone construct. One embodiment involves completion of a standard ligament repair, followed by the implant of anchors to secure sutures between bone locations proximal to the native origin of the repaired ligaments. Each of the sutures is implanted with at least some slack. Once the sutures are secured, the first suture is alternatively tensioned and moved through a range of motion, followed by the same process in a second suture. The steps are performed iteratively until the range of motion is controlled or limited sufficiently to protect the repaired ligaments.

The loop can be formed by disposing the second terminal end of the filament within a volume of a portion of the intermediate portion of the filament.     Abstract: A laparoscopic clip applier can have a geared clip feed mechanism and a direct jaw clamping mechanism to provide smooth operation over a clip application stroke of a movable handle. The geared clip feed mechanism can include an idler gear and two drive gear racks to advance a feed slider feeding a clip to a jaw assembly. The two drive gear racks can be positioned to provide different gearing during clip feed and firing portions of the clip application stroke, allowing efficient packaging of the clip feed mechanism. A spring can rapidly withdraw the feed slider once the clip has been fed and before clamping begins. The movable handle can have a direct connection to a clamping slider to enhance user feedback during clamping of the clip. After clamping the clip, the movable handle can also assist in returning the clamping slider to an initial position for firing another clip.

    Abstract: The present invention discloses a self-sealing cannula and methods of its use. The self-sealing cannula can be minimally invasively placed into the heart for drawing and/or returning blood with a self-sealing function at the interface of the blood access site. The disclosed cannula can be implemented as a single lumen cannula or a double lumen cannula, which can be used with ventricular assist devices for heart support or pump-oxygenators for ECMO and respiratory support. Through a self-sealing mechanism fixed on the ventricular wall or atrial wall, a cannula body is attached to the self-sealing fixture and blood is drawn into the lumen via an external pump and returned to the circulation system through a separate cannula. In the case of the double lumen cannula embodiment, the blood will be drawn into the drainage lumen of the double lumen cannula and returned through an infusion lumen at the desired location.
