The inner structure may include a fabric panel bearing a ridge intended to occupy the cleft separating the buttocks, with the fabric panel adhering closely to the buttocks. The inner structure may also or instead include a fabric scrotum guard which covers the rear of the scrotum and which separates the scrotum from the inner thighs and separates the right and left perineal folds where inner thighs meet the lowermost trunk. The scrotum guard may include adjustment structure for adjusting tightness of the fit with the Y formed at the scrotum and the entire perineum. The front of the traditional covering may have an opening for retrieving the penis which opening is arranged horizontally rather than in the traditional generally vertical orientation.

The base has a first longitudinal axis and a surface extending from a first side of the base in a first direction corresponding to a height of the surface, which has a second longitudinal axis with a constant angular orientation relative to the first longitudinal axis, is of a size greater than the anal orifice, is anatomically configured to contact only the perianal region and to not substantially enter the anal canal or substantially interfere with the birthing canal. The positioning member extends from a second side of the base in a second direction away from the first side, and includes a substantially rigid structure configured for gripping by a user to position and hold the device against the anal orifice.

The frame comprises a sub-clavicula support and a pelvic and pubic support connected by side members. The sub-clavicula support may be connected by a turnbuckle screw arrangement to the side members. The hernial truss is formed with a front member applied to the abdominal area of the patent, a rear member applied to the back of the patient, left and right waistbands attached to the front member, left and right crotch bands attached to the rear member, and a pressing pad attached to the inside of the front member. A lower portion of the rear member is divided to left and right by an upward notch. The front and rear members are fitted with the patient body by the waistbands and the crotch bands, and each crotch band connects the rear member and the front member through the bottom of the patient to press the pressing pad against the herniated abdominal area.
