The engaging instrument is retracted to move the engaged tissue into the stapling position, and driving staples from the cartridge through at least two layers of the engaged tissue. The method and associated system may be used to form plications in body tissue, such as stomach wall tissue. Staples simultaneously driven through tissue may simultaneously capture a reinforcing elements positioned adjacent the cartridge and/or anvil prior to stapling.

The actuator handle assembly has a first actuator that controls motion of the anvil and a second actuator that controls motion of the staple driver. The shaft assembly couples the stapling head assembly to the actuator handle assembly. The safety latch prevents operation of the second actuator when the gap between the anvil and staple holder is outside a predetermined range. The locking member is configured to prevent adjustment of the anvil gap after the desired staple height has been set inside the predetermined range.

    Abstract: Device and method for anastomosis may include a circular stapler with central anvil and multiple circumferentially disposed stapling limbs. A tubular graft may be sleeved on the anvil. The anvil with sleeved graft may be inserted into an end of a dissected aorta to form an overlap. Control actuation may cause the limbs to all substantially simultaneously close and then fire staples respectively into the overlap. The staples may form two generally parallel rows around the overlap with staples of each row being staggered relative to staples of the other row, thereby making a leak-free connection between the tubular graft and the end of the aorta. Control actuation may cause the limbs to open so the anvil may be removed from the end of the aorta, leaving the tubular graft in a leak-free connection with the aorta's end. Exemplary embodiments may allow for generally faster anastomosis than prior art.

    Abstract: Endoscopic plicators are passed transorally into the stomach and used to plicate stomach tissue by engaging tissue from inside of the stomach and drawing it inwardly. The tissue is drawn inwardly into a vacuum chamber, causing sections of serosal tissue on the exterior of the stomach to be positioned facing one another. The plicators allow the opposed sections of tissue to be moved into contact with one another, and preferably deliver sutures, staples or other means for maintaining contact between the tissue sections at least until serosal bonds form between them. Each of these steps may be performed wholly from the inside of the stomach and thus can eliminate the need for any surgical or laparscopic intervention. After one or more plications is formed, medical devices may be coupled to the plication(s) for retention within the stomach.

The staples may be fired through the cover member and the balance of the cover member is withdrawn from the patient with the stapling head. Other cover embodiments are selectively movable from a position wherein the distal face of the stapling head is covered and other positions wherein the distal face of the stapling head is exposed.

The surgical stapling instrument and/or disposable loading unit can further include a threaded collar and/or detent assembly configured to hold the disposable loading unit in place. In various embodiments, a disposable loading unit can include a lockout feature which can prevent, or at least inhibit, an expended disposable loading unit from being reassembled to the elongated body of the surgical instrument.

The self-closing feature of the staple does not lead to tissue crushing. 2. Edge seams are applied generally perpendicular to the tissue edge, allowing for blood supply to the tissue edge. 3. Excess tissue is cut off at the seam, minimizing the amount of tissue that may undergo local necrosis. A physical feature prevents the staple from rotating, when in an application shaft, to maintain a predetermined closing direction. The stapling element may be formed of an alloy, a pure metal, a polymer, or a composite of at least two materials. Additionally, staplers, anastomosis devices, and methods for their applications are described.
