Abstract: A computer system for identifying medical care providers outside a process-of-care standard for a field of health care is configured to perform steps that include retrieving claim line item information including procedure/service codes, and retrieving definitions for marker code groups associated with each of a set of medical conditions; deriving, for each marker-condition pair, an actual rate of utilization of the marker code group for episodes of the associated medical condition; and assigning a status to each marker-condition pair in response to the actual rate of utilization respectively exceeding or not exceeding a target rate. The steps also include aggregating the statuses across the marker-condition pairs to obtain an overall score, and causing an output to be displayed in a viewable format. The output includes the overall score, each marker-condition pair of the set of medical conditions, and the status for each marker-condition pair of the set of medical conditions.

Abstract: Methods and systems are provided for dynamically presenting visually codified information associated with healthcare claims for payment of medical services and/or products provided to respective patients. Received healthcare claim information is stored in a healthcare claim database, and a healthcare claim graphic is created for each healthcare claim according to pre-determined specifications and using the healthcare claim information. Each healthcare claim graphic includes data fields for displaying different categories of information, a color scheme for representing information included in each of a first group of data fields, and one or more symbols for representing information included in each of a second group of data fields. A collection of healthcare claim graphics is displayed with a plurality of zoom options for dynamically toggling between a summary view of the entire collection, a semi-detailed view of a group of graphics, and a detailed view of one graphic.

Abstract: A robotic system that can be used to treat a patient. The robotic system includes a mobile robot that has a camera. The mobile robot is controlled by a remote station that has a monitor. A physician can use the remote station to move the mobile robot into view of a patient. An image of the patient is transmitted from the robot camera to the remote station monitor. A medical personnel at the robot site can enter patient information into the system through a user interface. The patient information can be stored in a server. The physician can access the information from the remote station. The remote station may provide graphical user interfaces that display the patient information and provide both a medical tool and a patient management plan.

Abstract: The present invention relates to a system and method for automated extraction and display of past health care use to aid in predicting future health status. A system and method that converts raw medical and pharmacy claims data into Hierarchical Major Clinical Condition (HMCC) and Place of Treatment (POT) time-series data to facilitate the health assessment of a member's total clinical conditions and aid in predicting his or her future health status. The HMCC categories are organized in body systems and likely disease progression to permit both spatio-temporal digital signal processing and the development of a dynamical learning system. Each medical and pharmacy claim of the member is mapped onto one or more HMCC/POT-time cells. At the end of mapping, multiple entries in each HMCC-time cell are accumulated with the temporal resolution determined as a function of group size and temporal fidelity required for model building.

Abstract: A method and system for furnishing an on-line quote for an insurance product, such as auto insurance, makes use of computer hardware and software to provide a user with an on-line, real-time quote. The system allows the user to enter user information and recommends coverages, liabilities, and deductibles to help the user determine the best insurance coverage for the user's needs. Upon entering the user data and coverage information on a presentation server, deployable content profile information is retrieved by the presentation server, and a quote is displayed for the user. If the user indicates a continued interest in the quote, a client number is presented to the user and sent to a computer telephony integration database.

Abstract: An inventive process is disclosed for linking credentialing information with a medical malpractice insurance application. The credentialing information is automatically transferred from the credentialing questionnaire to an insurance application, and this credentialing information is then used to generate a medical malpractice insurance policy. The medical malpractice insurance policy is a two year policy, in order to coincide with the required re-credentialing of the healthcare provider. The inventive process also includes linking an information database, not created for insurance purposes, with an insurance application.

Abstract: Methods of transferring risk of costs for repairs, methods of providing funding for repair costs, and articles for transferring risk of repair costs. Articles may be service contracts, and the methods and articles may apply to one or more apparatuses, which may be motor vehicles, for instance. The methods and articles may include a right for the purchaser to renew a contract for one or more periods of time, or may include a provision to convert any unearned payment into extended funding for repair services in the event that anticipated payments are not made. A customer may pay for an initial term, after which the customer may renew for second or subsequent periods of time at the customer's option. A motor vehicle service contract may terminate after a delineated number of miles is reached, and an auto-renewal status may be reached after a particular number of payments are made.

Abstract: An information tag is provided that enables important vehicle insurance information and identification to be kept on a keychain or analogous object, for example. In one illustrative embodiment, an information tag includes a card, an aperture in the card, and a set of content disposed on the card, wherein the content is associated with an insurance policy. Additional embodiments also include a method for providing information tags, and a sheet that includes a plurality of detachable information tags.

Abstract: A method and system for generating, inventorying, storing and managing records and record forms is disclosed. The record management system and method utilizes a system record generator which generates a plurality of policies, record forms and the like in response to a questionnaire record. Customized record forms and policies are generated and stored in a unique record receiver having indicia for clearly identifying forms therein. A record holder is also provided for storing, inventorying, and managing the record forms used. The method and system are particularly useful for managing and insuring that record forms are completed for employees in a company by providing convenient means for visually identifying record forms, such as employment applications and the like, for ascertaining when one or more forms is missing by the presence of an empty compartment in the record holder.

Abstract: The method and apparatus of this invention permit quality review by medical insurers of ambulatory patient care by gathering medical data on each and every ambulatory visit and by providing a unique data transmission system to timely and accurately report the data for analysis. Negotiable medical data drafts are provided to participating medical care providers, who are authorized to issue the draft to themselves and sign the draft at the conclusion of each patient's visit. In exchange for immediate partial payment for services rendered, the medical service provider is required to enter the requested medical data summarizinq the patient's visit on the data entry portions of the negotiable medical data draft. Deposit in its bank of the medical data draft by the medical care provider returns to the provider immediate cash and places the medical data in a transmission system designed and monitored for accurate and reliable handling.

Abstract: A pocket size punch mechanism is provided to punch openings in cards or card-like objects, the punched openings have a cross-sectional configuration and spacing matching the configuration and spacing of a pair of rails in a commonly available card file. Also shown is mounting structure for adapting cards and card-like objects for mounting in the same type of two rail card file. A plurality of mounting members of a stiff stock material are fabricated on a sheet, the mounting members being individually removable from the sheet as needed to form a member having openings conforming to the configuration of the card file rails. A portion of the mounting member has adhesive thereon to secure to the back of the item to be mounted.

Abstract: Personal information is recorded on an information medium containing both visual images, such as a face image or fingerprint, and laser recorded data. The visual images are created on a piece of photographic material or eye readable laser recording material. The visual image material is adhered to a surface of a wallet-size card. A strip of laser recordable optical data storage material is also adhered to the card. After the strip is put on the card, a laser records personal information indicia on the strip in situ. The strip may be a reflective material of silver particles in a gelatin matrix, in which recording produces spots having a detectable difference in reflectivity. The card may be coated with a transparent protective laminate material.

Abstract: To reduce the cost of air travel insurance premiums substantially, by eliminating the need for insurance company representatives, agents, or machines, in airport terminals or elsewhere, purchasers contract with and prepay a nominal sum to the insurance company for a selected amount of air travel insurance coverage. The insureds are supplied through the mail with a designated number of proper documents in assembled relationship for use in coaction with conventional air travel tickets in a manner whereby the purchaser can assure the validation of his or her air travel insurance prior to take-off on a mode of air travel. A return envelope is provided, addressed to the insurance company or agent. The envelope includes a positioning means for the air travel ticket and insurance documents assembly to be temporarily assembled with the envelope to enable the purchaser to imprint the validating indicia on an element of the ticket to be given to the air travel ticket agent and also on the insurance documents assembly.

Abstract: One or more pages of a publication are provided with a pre-printed perforated area which may be removed from the remainder of the page or with a pre-printed form which may be peeled from the page. The perforated areas provide individual cards when removed from the page, while the peelable forms are placed on a backing when they are separated from the page. The cards and backings are provided with shaped cut-outs at one edge for placement in a suitable storage container.
