Abstract: A computer-implemented method for granting a user access to an on-line banking area of a website belonging financial institution responsive to receiving the valid user name and password. The method providing the user with account information regarding checking, savings, mortgage, home equity and/or loan accounts held by the user at the financial institution. The method grants the user access to a request tracking area of the website of the financial institution, receive request related data from the user, the request related data including data relating to at least one expense or goal, calculate portions of each expense respectively owed by each of a plurality of participants in the request and track whether a participant has paid. The method includes sending a message to at least one participant, the message requesting payment for the portion owed by the participant and receiving the payment from the participant in various payment forms.

Abstract: A system of color coded reference tables is disclosed by which fiber optic installation workers and engineers may quickly and easily convert fiber count numbers to bundle and sheath colors. The color code used may be an industry-standard 12-color or an alternative mapping of colors to numbers. The tables convert a numerical cable count having in principal no upper bound to an identifiable color code, which may be applied to cables and related equipment in the field. The tables of the system may be printed on a laminated card or page where an installation worker may make temporary notations using a solvent-erase marker (for example to check off fibers for which work has been completed).

Abstract: A multiple-ply label roll includes end-to-end labels. Each label is operable to be adhesively applied to a substrate and includes top and bottom label plies. The bottom label ply includes an outer removable border portion defined between interior and outer ply edges and removable from the top label ply in a removal direction. The interior ply edge includes spaced apart side edges that extend along the removal direction to define a pair of spaced apart side sections of the removable portion. The interior ply edge includes a generally transverse end edge interconnecting the side edges to define an end section of the removable portion, with the end section extending between the side sections and being removed generally after the side sections when the removable portion is removed in the removal direction.

Abstract: Disclosed herein are embodiments of a customizable surgical template and methods for preparing and providing the same. More specifically, the disclosed surgical template coordinates the arrangement of surgical instruments used for a particular surgical procedure in the order preferred by a particular surgeon or other entity.

Abstract: A multi-purpose form with a plurality of label types on a single business form includes an adhesive-backed label, a label with a front-facing exposed adhesive, and labels which can form laminated water-resistant tags. The business form has a plurality of layers. The form includes die cuts which allow for layers to be peeled away from the form, exposing adhesive layers and forming the different label types.

Abstract: A point of sale system uses change from cash transactions to, upon the approval of a customer, purchase a lottery stake. The system generates a receipt for both the point of sale transaction and the lottery stake purchase. The receipt indicates when the customer has won. In particular the receipt has imprinted a list of items purchased and a lottery indicator imprinted on the receipt for indicating whether the receipt is a winner. Ideally, the receipt is printed on paper and includes a subtotal, a tax line item, a total, and a change line item. At least a portion of the change line amount represents the lottery stake. In one embodiment of the invention, the receipt includes the statement: “You Won!”.

Abstract: According to an embodiment of the present invention, a computer implemented method and system for automatically implementing a fraud strategy may involve identifying transaction data related to a customer transaction based on a payment instrument; automatically identifying a pattern based on one or more factors associated with a customer spending profile; identifying a potential fraud situation based on the identified pattern and the transaction data; executing an action for the potential fraud situation; and adjusting authorized use of the payment instrument.

Abstract: A business form is provided for printing within a high-heat printer and transferring an impressed image between portions of the form. The form includes a first portion with a carbonless transfer coating applied to the rear face thereof, and a second portion with a carbonless transfer coating applied to the front face thereof. The portions are positioned in an edge-to-edge relationship adjacent one another, defining a seam therebetween, and are retained in such position with heat-resistant tape. The tape includes a perforation line that extends along the seam to facilitate folding therealong and easy separation of the portions from one another. The portions include indicia areas with preprinted indicia thereon positioned such that when the first portion is folded behind the second portion, the indicia areas and preprinted indicia precisely align and an impressed image created on the second portion is automatically transferred to the first portion. A method of forming the business form is also provided.

Abstract: A chain-of-custody form having integrated removable specimen seals and methods for using the form to identify a test sample collected from a donor and document the handling of the test sample. The form comprises a blank having a first portion and a second portion. The first portion comprises a first section having one or more spaces for recording demographic and testing information, and one or more indicia of the information to be recorded in one or more marked spaces; and a second section having a marked space for the donor to affix a signature and one or more indicia of the purpose of the space. An identifier that is unique to the form is recorded within the first portion and within the second portion. The identifier includes an specimen identification number or some other method of specimen identification such as a bar-code.

Abstract: A method and apparatus for printing and storing transaction records in a random manner is provided. A printer is coupled to a storage tray wherein the storage tray or the contents thereof are moved, shifted, or shuffled to randomize the storage of printouts in the storage tray and prevent the storage of sequentially generated printouts. A shifting mechanism, a blowing mechanism, or a combination thereof, is used to shift the storage tray or move or shuffle the printouts in a storage tray, and algorithms are used to set the motion of the shifting or blowing mechanism, or combination thereof.

Abstract: A system for providing a shipping label and a packing list on a container is provided which enables rapid and automated sequential application of a packing list and a shipping label to the container. The shipping label is configured complemental to the packing list whereby the packing list may first be adhered directly to the container and then the shipping label may be placed thereover. The shipping label may be applied over the packing list while the container moves continuously down a conveyor. The invention further includes an apparatus for sequentially applying the packing list and then the shipping label, and a method for applying the packing list and shipping label in registry.

Abstract: An expense receipt diary of a notebook-type (10) consisting of a front cover (12), a back cover (14), and a plurality of interior pages (16), secured together by binding (18) on one side. The front of each interior page (16) consists of a mounting area (24) that contains a plurality of adhesive strips (28). Each strip (28) is comprised of a release liner (30) and an underlying adhesive (32). Receipts (34a) may be attached to the underlying adhesives (32) by removing the liners (30) and pressing the receipt (34b) onto the exposed adhesives (32). The recording area (20) is located on the reverse side of the mounting area (24). The recording area (20) is a printed surface where the employee can record details pertaining to the receipts attached (34b) to the mounting area (24) of the same sheet.

Abstract: A mailing assembly for use in connection with non-domestic delivery of a mailpiece wherein the sender of the mailpiece is required to complete a sender's declaration. The assembly includes a mailing form that may be sub-divided into customs declarations forms, a dispatch notice and a backing sheet. The backing sheet may be peeled off the mailing form exposing adhesive on the backing sheet. The backing sheet may be folded in such a way as to provide an envelope to adhere to the mailpiece to be non-domestically delivered. The customs declarations forms and, dispatch notice may then be inserted into the envelope whereupon the mailpiece may be non-domestically delivered. The customs declaration forms and dispatch notice of the mailing form may be variably printed with information to aid in the non-domestic delivery of the mailpiece.

Abstract: A medical test form having a biological sample receiving material mounted thereto. In one embodiment, the sample receiving material is incorporated in a sample receiving sub-assembly that is secured to a multi-panel carrier substrate that has instructions and/or patient information printed thereon. In another embodiment, the sample receiving material provided on one panel of a V-fold mailer. If necessary or desirable one or more additional panels can be secured to the panels of the V-fold mailer.

Abstract: A reusable checklist device for providing a checklist which is erasable. The reusable checklist device includes a panel having a front surface, a back surface, an upper edge, a bottom edge, a first side edge, and a second side edge. The panel is substantially rigid. A plurality of number indicia is located on the front surface and extends between the upper and bottom edges. Each of a plurality of horizontal lines extends between one of the numerals and the second side edge. An erasable writing utensil is adapted for removably writing on the front surface. A sleeve has an open end and is attached to the front surface. The erasable writing utensil is extendable into the open end of the sleeve such that the erasable writing utensil is removably stored in the sleeve.

Abstract: A mailer type business form intermediate, and method of producing a mailer from the intermediate, effect folding in a novel manner in order to produce a business form that is particularly advantageous for providing a negotiable instrument, such as a bank check or money order. The intermediate has conventional components, but with first, second and third fold lines defining a sheet of paper comprising the intermediate into first through fourth panels. The fourth panel has indicia defining the negotiable instrument and at least one line of weakness separating the negotiable instrument from the rest of the fourth panel. Tacking adhesive patterns may be disposed substantially parallel to the top and bottom edges of the intermediate to hold the first panel second face to the second panel second face adjacent the top edge and second fold line, and to hold the fourth panel second face to the second panel first face adjacent the first and third fold lines, when the intermediate is folded about the fold lines.

Abstract: A system, method and device for selectively tracking expenditures against a total monetary amount. The device includes a support having a plurality of response areas for presenting information for selection, and printing indicative of one or more discrete monetary amounts, each amount a small fraction of the total monetary amount as a discrete budget unit. The printing is fixed to the support within each response area. The plurality of response areas and the printing are arranged on the support in a distributed fashion such that the aggregate of the representations of the discrete monetary amounts equals the total monetary amount. The arrangement of the printing and response areas provides a visual representation of the total monetary amount as a distribution of discrete monetary amounts. Expenditure amounts are tracked by canceling, for each expenditure amount, a corresponding amount of the discrete printed representations to approximate the expenditure amount.

Abstract: The invention is an account management system using an adhesive transaction receipt, provided at the point of transaction, which can be directly transferred to an account register. In this system, the adhesive transaction receipt contains a printed record of information relating to the transaction, such as the transaction type and the amount of the transaction. The adhesive transaction receipt is temporarily affixed to a more complete transaction receipt and includes an adhesive so that it can be permanently attached to the account register. The adhesive transaction receipt simplifies bookkeeping and eliminates errors by relieving an account user of the requirement of entering individual transactions by hand.

Abstract: A combination shipping label/invoice form, including a shipping label section detachably secured along a dividing border to an invoice section, the shipping label section bordered at a first edge by a first marginal strip and at a second edge by a second marginal strip, the invoice section bordered by a third edge and a fourth edge, the shipping label section being wider between the first and second edges than the invoice section between the third and fourth edges, such that folding the invoice section along the dividing border and against the shipping label section results in the invoice section lying between the first and second edges of the shipping label section, where the lower side of the first and second marginal strips is coated with adhesive.

Abstract: A method and system for generating, inventorying, storing and managing records and record forms is disclosed. The record management system and method utilizes a system record generator which generates a plurality of policies, record forms and the like in response to a questionnaire record. Customized record forms and policies are generated and stored in a unique record receiver having indicia for clearly identifying forms therein. A record holder is also provided for storing, inventorying, and managing the record forms used. The method and system are particularly useful for managing and insuring that record forms are completed for employees in a company by providing convenient means for visually identifying record forms, such as employment applications and the like, for ascertaining when one or more forms is missing by the presence of an empty compartment in the record holder.

Abstract: A bill comprises a first sheet of paper having two or more information panels and a remittance panel defined therein. Information (e.g., text) in the information panels is oriented in a first direction, and information in the remittance panel (e.g., text) is oriented in a second direction, perpendicular to the first direction. In one embodiment, first sheet comprises two information panels on a first side, and two information panels on a second side. A first information panel comprises summary information; second and third information panel comprises billing information, and a fourth information panel comprises advertising information. The bill is folded such that information in the information panels reads like a book (excepting the remittance panel which is generally torn off). In another embodiment, information in side-by-side panels comprise information in two languages, for example English and Spanish.
