Abstract: A carbonless form is provided that may be folded selectively into a duplicating state or a non-duplicating state. In accordance with one embodiment of the present invention, a carbonless form is provided comprising a form sheet, a transverse fold line, an assigned data entry portion, and an exposed data entry portion. The form sheet defines a front face, a rear face, a top edge, a bottom edge, and a pair of side edges. The transverse fold line extends across the form sheet to define a first panel and a second panel on opposite sides of the transverse fold line. The assigned data entry portion is defined on the rear face of the second panel. The exposed data entry portion is defined on the front face of the first panel. A CB coating is applied to the rear face of the form sheet in the first panel and a CF coating is applied to the rear face of the form sheet in the second panel.

Abstract: A mailer type business form intermediate, and method of producing a mailer from the intermediate, effect folding in a novel manner in order to produce a business form that is particularly advantageous for providing a negotiable instrument, such as a bank check or money order. The intermediate has conventional components, but with first, second and third fold lines defining a sheet of paper comprising the intermediate into first through fourth panels. The fourth panel has indicia defining the negotiable instrument and at least one line of weakness separating the negotiable instrument from the rest of the fourth panel. Tacking adhesive patterns may be disposed substantially parallel to the top and bottom edges of the intermediate to hold the first panel second face to the second panel second face adjacent the top edge and second fold line, and to hold the fourth panel second face to the second panel first face adjacent the first and third fold lines, when the intermediate is folded about the fold lines.

Abstract: A movable model object which resembles a racehorse has a model supported on a base having wheels that run on a field of a horse racing game apparatus. The model includes front and rear legs as movable elements and a power transmitting mechanism for transmitting rotation of the wheels to the front and rear legs. The power transmitting mechanism includes an endless belt for transmitting the rotation of the wheels to a rotatable shaft rotatably supported on a fixed frame of the model to move the front and rear legs.

Abstract: An apparatus and method is provided for imprinting duplicate copies of a credit card receipt using a single-ply roll of paper. The paper can be either coated on a first side with a carbonless coating that contains rupturable microcapsules, each filled with reactive, colorless dyes, and coated on a second side with a resin adapted to react with the colorless dyes, or coated on a single side with self-contained microencapsulated colorless dyes and reactive resins. To include transaction information printed on the credit card receipt as well as information written on the receipt by the customer (such as a signature and an indication of the amount of any gratuity), the transaction information is substantially duplicated in two separate fields each on a separate portion of the credit card receipt. At least part of the two separate portions are then aligned and presented to the customer for completion.

Abstract: A prescription pharmacy label comprises a face stock having top and bottom faces with a pressure sensitive adhesive on at least a major portion of the bottom face, and a release liner engaging the pressure sensitive adhesive. A CB coating is provided on a first portion of the top face, and a CF coating on a second portion of the top face, separated by a first fold line, and a third portion of the top face is uncoated. A second fold line is provided in the release liner so that the label may be Z-folded about the fold lines so that the CB and CF coatings are in face-to-face engagement, and underlie the third portion, so that handwriting provided on the third portion is transferred to the CF coating on the second portion. A slit may be provided between the first and third portions to allow part of the adhesive on the bottom face of the third portion to be exposed and to be moved into contact with a prescription medicine container (e.g.

Abstract: Improvements to self-replicating duplex forms are disclosed. Generally, a single sheet of paper is divided into original and copy panels by fold line, and carbonless coatings are applied to the panels so that information entered on the two, front and back surfaces of the original panel are reproduced on the two surfaces of the copy panel.(Group 1) An endorsable carbonless CB coating is applied to the original panel, either at the mill or on-press. A carbonless CF coating is applied to the copy panel so that the coated copy panel is substantially the same thickness as the coated original panel. Specific areas for filling out information on the original panel are offset, from front-to-back, and methods of checking this offset are disclosed. The copy panel is tinted a dissimilar base color from the original panel.(Group 2) The original and copy panels can be formed separately, and joined into a single "virtual" sheet.(Group 3) The original and copy panels can be formed of a single sheet of CF C2S paper stock.

Abstract: A method for folding paper into letters of the alphabet. The method has defining a rectangular piece of paper with an X-axis paralel to a width thereof and a Y-axis parallel to a length thereof, folding the piece of paper into the shape of an alphabetic letter at lines parallel to the X-axis, thereby dividing the piece of paper into a plurality of segments, in segments to be formed into curves, defining a plurality of alternative lines X' and X" parallel to the X-axis so that a sub-segment of paper between every two sequential lines X' and X" is longer than a sub-segment of paper between every two sequential lines X" and X', corrugating the piece of paper at the alternative lines X' and X", corrugating the piece of paper at lines parallel to the Y-axis, and bending the folded and corrugated piece of paper into an alphabetic shape.

Abstract: Various configurations of "clear" areas for entering information on the front and back sides of an "original" page of a two-sided form are disclosed. The page may be the original page of a self-replicating form which includes a copy page and appropriate carbonless coatings. The clear area on the front side of the original page is offset from the clear area on the back side of the original page. A method for laying out the original page of the form, such as with a computer system, to ensure offset of the front and back clear areas, is also disclosed. A method of filling out "variable" information in the offset clear areas, such as with a computer system, is also disclosed.
