Abstract: A retro guidewire reamer includes a cutting member, and a mechanism for moving the cutting member from a closed position to a deployed position in a single manual motion. Once a desired size of a bone tunnel is established, a surgeon uses the reamer to create a primary bone tunnel over a guidewire from the outside in. The surgeon retracts the guidewire, and activates the mechanism to deploy the cutting member within the bone joint to conform to the size of a tendon graft. The surgeon uses the deployed cutting member to create a counter bore through the bone in a retrograde manner. Once the counter bore is drilled, the surgeon activates the mechanism to close the cutting member, allowing the reamer to be withdrawn through the primary tunnel. The retro guidewire reamer can be used to provide more accurate bone tunnel placement during ligament reconstruction surgery.

The bone engaging side can include a profile for positioning in the trochlear groove and the guide holes can include axes that can be orientated at an acute angle. Retaining members can be positioned in bores formed in the body and can fix the guide to the femur. A cutting device can be guided by the guide holes and can include a bone engaging end having a body and first and second cutting portions. The first cutting portion can have a smaller diameter than the second cutting portion and can be axially spaced apart therefrom so as to form a stepped pocket in the femur. The body can include an outer diameter sized to be complimentary to an inner diameter of the guide holes.

Abstract: Methods and devices are provided for forming holes in bone to stimulate bone growth. In one exemplary embodiment, a surgical instrument set can include a driver having a first mating element, and a bone pick have a second mating element configured to releasably mate with the first mating element at any of a plurality of angular orientations. When releasably mated together, the driver and the bone pick can be configured to be adjustably positioned relative to one another with three degrees of freedom, e.g., pitch, yaw, and roll. The bone pick can include a piercing tip configured to form a hole in a bone, such as in microfracture. When the driver and the bone pick are releasably mated together, the driver can be configured to be struck, impacted, or otherwise manipulated to drive the piercing tip into the bone.

Abstract: The present invention features new methods, apparatuses and devices for fixing adjacent bone segments, segments of a bony structure and adjacent vertebrate of a spine. The methods, apparatuses and devices utilize a new apparatus for forming a channel in a surface of the bone or bony structure segments or adjacent vertebra or a channel submerged within the bone or bony structure segments or adjacent vertebra. In more particular embodiments such apparatuses and methods including forming an arcuate channel and which channel can receive therein a curved rod or implant member, which also preferably is arcuate, and avoids the associated problems with prior cage or straight rod and screw systems. Also featured are systems, apparatuses and methods for repairing, replacing or augmenting the nucleus and/or annulus of a disc.

Abstract: Tissue removal probes comprise an elongated member, a drive shaft rotatably disposed within the member, and a rotatable tissue removal element mounted to the distal end of the drive shaft. One tissue removal element comprises a plurality of tissue-cutting filaments affixed at proximal and distal ends of the tissue removal element. The cutting filaments may have optional hinge points that allow the distal end of the tissue removal element to be inverted, thereby transforming the tissue removal element from a tissue-cutting device to a tissue-grasping device. Another tissue removal element may have a blunted tip to prevent distal tissue trauma and an irrigation port to provide irrigation fluid to the removed tissue and/or tissue removal element. Another tissue removal element has a proximal and distal spiral grooves that are oppositely pitched, so that removed tissue can be collected in the middle of the tissue removal element.

Abstract: A system for anterior cruciate ligament (ACL) reconstruction using holes in bone created by retrograde cutting. A rotary cutter, mounted onto an insertion post of a tibial guide, is inserted through an anteromedial portal and the rotary cutter placed on the anatomical origin of an ACL tibial insertion. A drill pin is drilled through the tibia and advanced until it contacts and engages a cannulation in the rotary cutter on the guide. Further rotation of the drill pin disengages the rotary cutter from the guide. The retrograde drill pin is then retracted and simultaneously rotated for retrograde cutting of a socket or tunnel of desired depth in the tibia. A similar system is provided to form a socket or tunnel in the femur except that a femur guide is used in place of the tibial guide.
