The system identifies a set of objects from the electronic form to be included within a panel used by a wizard to complete the electronic form. The system generates the panel for presentation by the wizard. The panel allows the user to provide input to those objects on that panel.

Abstract: A system and method for creating a table from one or more form images is described. An Image Based Document Management (IBDM) server includes a stroke identification module, an image generator, a table generator and a preference module. The stroke identification module receives completed form data from one or more forms with handwriting fields, media fields and symbolic fields. The image generator generates field images that are a combination of handwriting fields, media fields and symbolic fields. The user preference module determines a user preference for displaying the table. The table generator generates a table from the field images based on the user preference.

Abstract: A template processor converts an initial template document into an XSL template that can be used to automatically generate reports from report data extracted from a database or other application. Users create initial template documents using a word processor or other rich-text editing application. The initial template document employs the word processor's native formatting features to specify the formatting of report data. The initial template document includes placeholder tags inserted by the user to define the locations of report data in the report. The initial template document also includes transformation tags that dynamically define the content of a report based on the value or quantity of report data. The template processor can include a reader adapted to read portions of the initial template file and a parser adapted to create corresponding XSL code from these portions. The XSL template can be combined with XML-formatted data to create a report.

The form template information includes a first input field type selection and first field location information for the first input field. The processor stores the form template information within a memory. The electronic form is configured to be rendered by superimposing a representation of the first input field over the image of the form.

Abstract: An object template system is described herein that allows an administrator of an IT management application to define templates that provide a streamlined mechanism for pre-populating IT objects with common values based on business logic and processes. The object template system addresses manipulation of objects both as part of a user interactive session as well as in programmatic workflows, while also allowing for referencing of existing objects to assume integrity of data. A metadata-driven schema allows for portable definitions of simple and complex extensible objects. The system validates these definitions against the extensible types that define the objects to ensure structural integrity. Users can then apply these definitions at runtime to both create and/or update simple and complex management objects. Thus, the object template system allows an administrator to define controlled processes for creating and modifying management objects within a flexible IT management application.

Abstract: A system and method for providing an easy-to-use interface for verifying semantic tags in a steering application in order to generate a natural language grammar. The method includes obtaining user responses to open-ended steering questions, automatically grouping the user responses into groups based on their semantic meaning, and automatically assigning preliminary semantic tags to each of the groups. The user interface enables the user to validate the content of the groups to ensure that all responses within a group have the same semantic meaning and to add or edit semantic tags associated with the groups. The system and method may be applied to interactive voice response (IVR) systems, as well as customer service systems that can communicate with a user via a text or written interface.

Abstract: A referer verification apparatus and method for controlling web traffic having malicious code are provided. In the referer verification method, whether a referer is present in a Hypertext Transfer Protocol (HTTP) packet is determined. If it is determined that the referer is present in the HTTP packet, Uniform Resource Locators (URLs) are extracted from a referer web page corresponding to the referer. The referer is verified based on a URL corresponding to a referer verification request received from a server and the extracted URLs. A Completely Automated Public Test to tell Computers and Humans Apart (CAPTCHA) verification procedure conducted by a user is performed based on results of the verification of the referer.

The system includes a server having a data interface for receiving a question to be answered in the document. A data store associated with the server includes a plurality of directive words and descriptive text associated with each of the directive words. The server also includes a processor and associated memory, including program code for identifying directive words in the question and retrieving, from the data store, descriptive text associated with the directive words. An output interface then displays the descriptive text for each of the directive words.

Abstract: A system and method for paper-based, automated completion of forms allows a user charged with filling out multiple forms containing some common information to write each unique piece of information one time, and the system generates a set of completed forms with the required information on each form with no change to the use or structure of the current forms. The control module is operable on a processor to determine a mapping between common fields on multiple forms or multiple pages of the same form, copy information written in a field on one form to equivalent fields on other forms, and produce a set of completed forms. The system is computationally simple and independent of language because these operations can be performed without having to understand, translate or convert the writing. Furthermore, there is no loss of security or privacy beyond the use of paper forms because these operations can be performed without access to a centralized database and without connectivity to other systems.

The method includes: identifying a set of inputs to a form from past users; receiving, from a current user, a request to view the form; identifying temporal data associated with the current user; identifying an internet protocol (IP) address of a computing device of the current user; performing, based on the IP address and the temporal data, a statistical analysis on the set of inputs; determining, based on the statistical analysis, that a statistical threshold corresponding to a field on the form is exceeded; calculating, in response to determining that that the statistical threshold is exceeded, a default value for the field; and sending the default value to the computing device.

Each data entry element includes a set of information attributes describing the data entry element. The data entry elements of the data entry form are defined as one or more classes within the retrieved template. The data entry form is rendered by enumerating each of the one or more classes of the stored template and generating each data entry element of the one or more enumerated classes as described by the set of information attributes.

The contextual information helps to explain and/or give meaning to the user writing. Contextual information may include a descriptive name, an identification of data type, healthcare classification information, taxonomic information, an indication of the patient, an indication of the person that wrote on the form, and/or an indication of the date and time at which the writing occurred. The contextual information is then used by an Electronic Medical Record (EMR)/Electronic Health Record (EHR) application to perform a function such as patient/insurance billing, case management, or order fulfillment.
