Dielectric slabs may be used to extend the peak field region between top and bottom conducting surfaces to allow for more uniform exposure of planar materials that have a significant thickness. The catheter comprises an elongate tubular member having two coolant circulating lumens and an optical waveguide coextensive therewith, the proximal end of the optical waveguide being adapted to receive light from a light source and the distal end of the optical waveguide terminating in a diffuser tip. The diffuser tip provides a means for radially distributing treatment light from the light source to tissue adjacent to the distal tip of the catheter to heat the tissue. A transparent, inflatable balloon surrounds the diffuser tip. The inflatable balloon permits the flow of an optically transparent coolant around the diffuser tip to cool the tissue immediately adjacent to the diffuser tip to prevent overheating. A strip of components is formed which can be individually easily separated at severance regions between the adjacent components. The severance regions are located in the carrier links between the components. The carrier regions (21) that remain after severance may also serve as an overtravel stop during insertion of the component into a support. In a two-row embodiment (FIG. 10), the carrier regions (50, 52) are formed to be asymmetrical in order to increase spacing between pins. The holes in each set are longitudinally spaced from each other at substantially regular intervals along the length of the catheter. Each successive hole in a given group is circumferentially spaced by an angular distance of about 90.degree. around the circumference of the catheter relative to the immediately preceding hole. Each group of holes is circumferentially spaced or offset by an angular distance of between 1.degree. and 89.degree. relative to the immediately preceding group of holes. The amplified signal is then passed through a multi-level logarithmic attenuator having at least one fixed attenuation reference point to create an attenuated version of the detected and amplified ultrasonic signal.
