The first and second lens are arranged next to one another in a direction transverse to the first optical axis to provide no overlap in a direction along the first optical axis such that the first lens and the second lens each, independent from one another, image onto the macula of the eye. The angle and a direction of the second optical axis are chosen such that the second lens images onto a functional part of the macula of the human eye, which functional part is not compromised by a defect, such as a scotoma.

A plate is installable onto the rear sidewall of the polymeric spacer. An elevated screw support extends upwardly or downwardly from the plate so as to extend opposite the direction of the angled screw aperture. The elevated screw support locates a second screw to enter a second vertebrae through an anterior surface of the second vertebrae.

Prior to terminating the surgery, the opening formed by the removed vertebral corpus is fitted and/or filled with a suitable implant.

The contour of the alignment surface is complementary to a portion of the patient anatomy in a unique orientation that aligns the guide hole with tissue suitable for receiving a fastener. The alignment structure preferably includes an arm with a first end coupled to a rim of the guide and a second end coupled to the contour. In certain implementations, the guide includes a plurality of alignment structures. Further disclosed are methods of making and using a surgical guide for aligning an orthopedic implant.
