  • Patent number: 11335445
Abstract: A care plan risk rating (CPR2) system and method enables physicians and other clinicians to manage patient health care plans remotely in response to patient input to generate CPR2 values. In one aspect, patients under care for a particular condition respond to a series of questions related to that condition. The questions may be weighted according to predetermined criteria which a physician or clinician may set. Based on the CPR2 values, the physician or clinician may alter a care plan for a patient or a group of patients. In one aspect, patients with CPR2 values in a predetermined range may be grouped into a cohort, and their care plans managed as a group.

  • Patent number: 11315673
Abstract: Comprehensive molecular profiling provides a wealth of data concerning the molecular status of patient samples. Such data can be compared to patient response to treatments to identify biomarker signatures that predict response or non-response to such treatments. This approach has been applied to identify biomarker signatures that strongly correlate with response of colorectal cancer patients to FOLFOX. Described herein are data structures, data processing, and machine learning models to predict effectiveness of a treatment for a disease or disorder of a subject having a particular set of biomarkers, as well as an exemplary application of such a model to precision medicine, e.g., to methods for selecting a treatment based on a molecular profile, e.g., a treatment comprising administration of 5-fluorouracil/leucovorin combined with oxaliplatin (FOLFOX) or with irinotecan (FOLFIRI).

The regimen specifies a total amount of basal insulin medicament, one or more basal injection event types for a recurring period, and an apportionment of the total amount of medicament between the injection event types. Time stamped glucose measurements of the subject are obtained over a past time course comprising a plurality of instances of the recurring period. When the glucose measurements satisfy a stop condition, a recommended adjustment is determined that comprises a change in the number of injection event types in the regimen and/or a change in the apportionment of insulin medicament between injection event types. The recommended adjustment is communicated to the subject for manual adjustment of the regimen, an insulin pen charged with delivering the regimen to the subject, or a health care practitioner associated with the subject.
