    Abstract: A system that can enable establishment of a self-describing data network is provided. Generally, the innovation provides a mechanism by which self-describing data can be collected, validated and stored in such a way that permits each data element to be inherently self-describing. The manner in which the data is stored can be analogized to a ‘data chemistry’ whereby data is stored in the smallest meaningful bit (e.g., atom) coupled with descriptive metadata (e.g., tags). In a specific example, the data network maintains health-related data where each element includes a core data element wrapped with descriptive metadata. The descriptive metadata (e.g., tags) can be employed to interrelate the data elements for storage as well as to facilitate efficient traversal of the data network as a whole.

    Abstract: Systems and methods are provided to limit exposure of medical Plans to high liabilities for renal dialysis without exposing the insured patient to out-of-pocket liability while still allowing the patient to receive dialysis as they need it. In one embodiment, this goal is achieved by providing an ERISA medical insurance Plan structure that limits Plan responsibility to 125% of the Medicare approved amount for the services provided, and simultaneously supplements coverage of the patient under Medicare Part B. Because Medicare regulations prohibit providers from balance-billing patients subsequent to billing Medicare, the patient cannot be billed for amounts in excess of the Plan limits.

The information relating to desirability of an intervention in a plan participant's health care regimen, as well as a plan participant's medical information and claim history, is presented to case managers and/or health care providers in a user-friendly format.

When the case is ready for submission, members of the medical professional team are selected for consultation on the case and forwarded the particulars of the submitted case via the Internet. The selected team members review the submitted case particulars and each member provides their medical opinion via Internet. The submitted opinions are recorded and displayed in real time on the system website, to which the user has access. In addition, the submitted opinions to structured questions are compiled to determine if there is a consensus.
