    Abstract: Some pathological conditions are accompanied by the increase in dimers of free light chains (FLCs) kappa and/or lambda. For instance, high levels of FLC dimers are found in AL amyloidosis, multiple myeloma, and multiple sclerosis. There is thus a need for sensitive methods that could specifically and quantitatively measure the amount of FLC dimers in a sample. The present application relates to a method for detecting and quantifying immunoglobulin FLC dimers in a sample that comprises subjecting the sample to proteolytic digestion under non-reducing conditions and subjecting the digested sample to mass spectrometry analysis to detect immunoglobulin free light chain dimer peptides. The method may be useful for diagnosis and monitoring of diseases characterized by the aberrant production of FLCs, such as plasma cell discrasias. Light-chain deposition disease (LCDD), autoimmune diseases, chronic kidney disease (CKD), and inflammatory conditions.
