A metal plate lamination hub member is inserted into and fixed in a central hole in the substrate of the disk. The metal plate lamination member is formed of a metal plate and a resin hub member having center holes dimensioned and aligned such that the center hole in the metal plate forms the center hole of the disk. The metal plate and the resin hub member are attached to one another by ultrasonic welding. A transparent circular substrate is provided having a large circular hole. A metal lamination disk member is formed having an outer diameter larger than the inner diameter of the hole in the substrate and an inner diameter smaller than the inner diameter of the hole in the substrate. The metal lamination disk member is placed on the substrate with an adhesive and positioned so that the center hole in the lamination member is concentric relative to the substrate, whereupon the adhesive is hardened.
