    Abstract: A medical information (“MI”) system includes a networked server (10) providing information to patients (441) and physicians (444) prior to, during, and following an office visit. The patient connects to the MI system site, which interactively interviews the patient regarding the condition for which the patient will be seen by the physician. The MI system site generates pre-visit information including an editable preliminary chart note (180). A pre-visit physician report may include expert medical information relating to the patient's condition(s), such as differential diagnoses, work-up algorithms, treatment considerations, and literature references. A pre-visit patient summary (145) relating to the patient's condition, including key questions are provided to the patient. The pre-visit physician report may include these key questions and a rationale for each question.
