The form construction is desirably a composite form having at least first and second discrete portions one of which carries one or more removable elements and the other of which provides an information carrying portion. The business form construction of the present invention includes a bonding material that secures the first and second portions to one another in an adjacent manner. The bonding material is covered with a coating which prevents peppering or tracking of toner during subsequent processing through a laser printer or ion deposition print engine.

    Abstract: The present invention relates to a discontinuous business form construction created from differential substrates that has been provided with one or more removable elements such as cards, labels, tags and differential substrates. The form construction is desirably a composite form having at least first and second discrete portions one of which carries one or more removable elements and the other of which provides an information carrying portion. The form construction of the present invention is produced in such a manner so that when a series of the forms are placed into a stack, such as in a tray that supplies forms to a printing station or a laser printer, there is no significant pad lean or distorted stack arrangement. That is, the stack of forms retains a relatively square, rectangular or cube shape as opposed to one having a sloped configuration, a high end and a low end, due to differential thicknesses between the two portions of the construction.

    Abstract: The present invention relates to a composite form and a method of printing the form. The composite form has a paper component and a flexible magnetic sheet component that are adhered to one another. The magnetic component is able to adhere to a ferromagnetic surface but does not adhere to other composite forms having a magnetic component through use of a shielding layer when the forms are provided in a stacked configuration for feeding into a printer. The composite form may be fed through a printer without jamming or magnetic attraction to components within the printer. The composite form is constructed to facilitate printing by having a single thickness entering the printer with a small area of overlap between the paper component and magnetic sheet component or the magnetic component may be inlaid into an opening in the paper component.

    Abstract: A form is provided which is especially adapted for printing which includes a carrier sheet having a backing sheet and a matrix sheet and at least one card adhered to the carrier sheet and positioned within an opening in the matrix sheet. The card is imprinted with indicia and the portion of the backing sheet visible within the opening of the matrix sheet when the card is removed is printed with an image similar or the same as the indicia so that when the card is removed, the information or impression conveyed by the indicia is still seen on the carrier sheet. The card is preferably removably adhered to the backing sheet by a layer of adhesive. The card may include a magnetic element so that when removed, it is self supporting when placed on a ferromagnetic surface. The upper surface of the card, when positioned on the carrier sheet within the opening, is preferably substantially co-planar with the top side of the matrix sheet.

    Abstract: A composite form and method of printing the same are provided, the composite form having a paper component and a magnetic sheet component of flexible magnetic material adhesively attached to the paper component. The magnetic sheet component is magnetically influenced to an extent which permits the magnetic sheet material thereof to be self-sustaining when the back face of the material is placed on a vertical ferromagnetic surface but not to magnetically adhere to other of the composite forms when stacked in a tray for feeding into a printer such that the composite forms individually feed into a laser or ink jet printer without jamming. The magnetic sheet component includes a synthetic resin laminate on the front face which helps to shield the magnetic field. The magnetic sheet component is preferably arranged relative to the paper component so that the full thickness of the magnetic sheet component does not overlap the paper component.
