The substrate includes a connection portion and extension portions respectively connected to the connection portion. A gap is positioned between any two adjacent extension portions, and a width of the gap is greater than or substantially equal to a width of any of the two adjacent extension portions. The conductive layer is disposed on the substrate. The light sources are disposed on the extension portions.
