    Abstract: Disclosed is a trocar, comprising a trocar sleeve, a sealing structure and obturator, wherein the trocar sleeve comprises a bowl-shaped proximal end housing part, a distal end portion and a sheath portion between the two; the sealing structure comprises a top shell and a bottom shell integrated with each other; and the obturator comprises a central shaft and a proximal end portion which is located at the proximal end of the obturator designed to be held by a surgeon; the axial direction of the trocar is defined by the extension direction of the central shaft which also has an obturator tip at the distal end to perform puncturing; the central shaft runs through the sheath portion of the trocar sleeve and the sealing structure in a sealed manner and the obturator tip of the central shaft extends out of the distal end portion of the trocar sleeve when the trocar is assembled.

The acoustic assembly comprises an acoustic waveguide coupled with the blade, a piezoelectric transducer element, a fastener, and a coupling member. The transducer element defines an inner diameter surface and an outer diameter surface. The fastener is configured to secure the transducer element relative to the waveguide. The coupling member is configured to provide electrical continuity between the fastener and the inner diameter surface of the transducer element. The outer diameter surface of the transducer element includes an annular recess. Another coupling member is configured to provide electrical continuity between the annular recess of the piezoelectric transducer element and a power source while permitting the piezoelectric transducer element to rotate relative to the body.

    Abstract: An instrument for clamping a surgical clip has a handle assembly, a shaft assembly, and an end effector. The handle assembly has a trigger and an articulation knob. The shaft assembly has an outer ground tube, a middle articulation tube, and a push/pull tube disposed within the middle articulation tube. The trigger is operable to cause longitudinal translation of the push/pull tube. The outer ground tube and the middle articulation tube present mating angled faces sloped in opposing directions. Rotation of the articulation knob causes rotation of the middle articulation tube. Rotation of the middle articulation tube changes the orientation of the angled faces and thus causes articulation of the middle articulation tube and the push/pull tube. The end effector has a pair of jaws configured to apply a surgical clip to a vessel. Longitudinal translation of the push/pull tube causes closure of the pair of jaws.

The transducer assembly is rotatably mounted along a longitudinal axis within the body such that the transducer assembly is configured to selectively rotate. The arrester is configured to move relative to the ultrasonic transducer assembly between an engaged position and a disengaged position and has a catch portion and a deflectable portion. The catch portion is configured to seize the transducer assembly and selectively inhibit rotation for rotatably coupling with the acoustic waveguide up to a predetermined torque. The deflectable portion is configured to deflect relative to the transducer assembly upon receiving a torque greater than the predetermined torque. Accordingly, the catch portion releases the transducer assembly to slip relative to the catch portion for limiting coupling of the transducer assembly with the acoustic waveguide to the predetermined torque.

The surgical instrument may also include an integral torque wrench for coupling the acoustic waveguide with a predetermined torque and an integral torque indictor for signaling to a user that the acoustic waveguide is coupled with the predetermined torque.

    Abstract: In various embodiments, a surgical instrument for operation in an aqueous environment is provided. In at least one embodiment, the surgical instrument may include a hollow sheath and a blade disposed at least partially within the sheath. Coupled to the blade may be at least one ultrasonic transducer, which, in turn, may be coupled to a drive system. The drive system may be configured to deliver gross axial motions to the blade such that the blade translates with respect to the hollow sheath when the drive system is activated. Accordingly, tissue may be cut by the blade with gross axial movement of the blade and/or ultrasonic vibrational motion provided by the ultrasonic transducer(s). In alternative embodiments, the blade may be rotated axially instead of translated with respect to the hollow sheath.

In one example a liquid dispensing feature provides a flow of cooling liquid to an ultrasonic blade via a lumen in a waveguide of the instrument. In another example, the blade has a serrated edge with a plurality of teeth arranged in a pattern to provide dimension and space during cutting actions. In another example, the blade has an oversize distal portion that creates an oversize slot when cutting to avoid excessive contact between the cut bone and proximal portions of the blade. In another example the blade includes an irrigation slot and also a micro slot in a distal end of the blade that connects with the irrigation slot.
