In FIGS. 8, 9, 15, 16, 22, 23, 29, 30, 36 and 37, the dashed broken lines encircling portions of the claimed design that are illustrated in enlargements form no part of the claimed design. The dot-dash broken lines represent the boundary of the claimed design and form no part of the claimed design. The legends included in FIGS. 14, 21, 28, and 35 that indicate the type of surface shading used to indicate the different materials included in the respective embodiment form no part of the claimed design. The back of the door ladder and shelf combination is flat and unornamented for all of the provided embodiments.

We claim the ornamental design for a door ladder and shelf combination, as shown and described.
