FIG. 2 is a top plan view thereof;

FIG. 3 is a front elevational view thereof, the rear elevational view being a mirror image of that shown;

FIG. 4 is a right side elevational view thereof, the left side view being a mirror image of that shown;

FIG. 5 is a bottom plan view thereof;

FIG. 6 is a bottom plan view of the operator mouthpiece structure, the operator mouthpiece is shown, separated for clarity of illustration;

FIG. 7 is a front elevational view of FIG. 6;

FIG. 8 is a right side elevational view of FIG. 6; and

FIG. 9 is a top plan view of the patient mouthpiece structure, the patient mouthpiece structure is shown separated for clarity of illustration.
