FIG. 1 is a front elevational view of a business form showing my new design, the undisclosed rear being plain;

FIG. 2 is a side elevational view thereof, the opposite side elevational being a mirror image;

FIG. 3 is a front elevational view of a second embodiment of FIG. 1, the undisclosed rear being plain;

FIG. 4 is a side elevational view thereof, the opposite elevational view being a mirror image;

FIG. 5 is a front elevational view of a third embodiment of FIG. 1, the undisclosed rear being plain; and,

FIG. 6 is a side elevational view thereof, the opposite elevational view being a mirror image.
