FIG. 5 is a cross-sectional view taken along lines 5--5 of FIG. 2; and,

FIG. 6 is a perspective view of a lamellar corrective insert without scale markings on the top surface thereof.

The bottom plan view, side elevational view and cross-sectional view of the embodiment of FIG. 6 are identical to the embodiment of FIG. 1.
