FIG. 1 is a front elevational view of a multi-label mailing form sheet showing my new design; and,

FIG. 2 is a rear elevational view thereof.

It is understood that the sides of the sheet is of no substantial thickness. The dashed lines shown in the above figures indicates perforations and are intended to be part of the invention.
