FIG. 5 is a right side view of the pharmaceutical dosage form shown in FIG. 1; and,

FIG. 6 is a left side view of the pharmaceutical dosage form shown in FIG. 1.

The dashed lines in FIGS. 1 to 6 represent environmental structure and form no part of the claimed design.
