Description

FIG. 1 is a front elevation view of the military data form showing my new design; and,

FIG. 2 is a rear elevation view thereof.

It is understood that the sides are of no substantial thickness. The outer rectangular broken lines shown in FIGS. 1 and 2 are for illustrative purposes only and form no part of the claimed design.

The outward radiating lines and the arch crossing the radiating lines that are shown in broken lines do form part of the claimed design.

The ornamental design for a military data form, as shown and described.
