Abstract: A carrier sheet comprising a printed paper sheet having an integrated detachable card with a magnetic backing and method of manufacturing same is described. The carrier sheet is comprised of a printed paper sheet having printed matter on at least a front face thereof. The printed matter contains card information printed in a card area. A polyfilm patch is adhered over the card area and over the card information printed thereon. A thin flexible patch of magnet material is adhesively secured on a rear face of the paper sheet and extends over the card area. A die-cut is made in the sheet and delineates the contour of the card area and extends through the polyfilm patch, the paper sheet and partly into the patch of magnet material to form a burstable region along said die-cut to hold the card in the carrier sheet. The die-cut can have any ornamental shape.

Abstract: A calendar is provided for behavior modification as well as a method for using the same. The calendar has calendar sheets and informational sheets. The calendar has objectives sections, data collection sections, analysis sections, non-date pages and/or detachable pages for use by an individual. The calendar may be customized and/or pre-designed for an individual or by the individual. In use, the behavior information may be recorded on the calendar sheet. Information associated with daily, weekly, and/or monthly behavior may be recorded. The information may include weekly and/or monthly averages and/or summations, delta calculations, and/or minimum/maximum data. The calendar may be adapted to measure or track indicators, such as, for example, developmental progress, caloric intake, amount and type of exercise, amount of fat, sodium or sugar, amount of cigarettes smoked, blood pressure measurements, weight gain or the like. A bottom space may also be provided to record monthly goals and/or objectives.

Abstract: A pamphlet (10) constructed in accordance with the principles of a preferred embodiment of the present invention and configured for receiving and storing a card (12) is disclosed. The illustrated pamphlet (10) broadly includes a single printable blank (14), printed with indicia (16), and folded to form an envelope section (18)—for receiving and securely storing the card (12)—and an adjacent panel section (20) that overlies the card-laden envelope section (18). The card (12) is preferably a key card, such as a hotel or motel room key. The blank (14) is preferably generally rectangular in configuration. The printed indicia (16) preferably includes a map (30) of the city in which the hotel or motel is located. The envelope section (18) is formed without the need for adhesives and is normally closed on three margins (18a, 18b, and 18d) and the fourth margin (18c) is closed by folding the panel section (20) into the closed position.

Abstract: An assembly for mailing an article requiring delivery by a special service and a method for mailing same are provided. The assembly includes a return postcard and anchor portions removably attached to the return postcard. The anchor portions include adhesive that allows the anchor portions to be attached to a mailpiece. The return postcard includes a special services designator section indicative of the special service required for delivery of the mailpiece. The return postcard is integrally formed, but removably attached, such that the return postcard remains attached to the envelope until received by the addressee, at which time the return postcard may be removed. The assembly is designed to incorporate a form into the return postcard to simplify preparation of the mailpiece for delivery by the special service. Alternatively, the postcard may contain a machine readable code that aids in the delivery and tracking of the mailpiece.

Abstract: Activatable point of sale cards in which multiple cards can be mounted on a display sheet for use in display or shipment applications. Several cards can be included in the package, with each card being associated with a unique account. The card accounts may be conveniently activated by orienting a data field on the display sheet that is associated with the accounts which can be used to activate all of the accounts in one transaction.

The promotional form may include a plurality of detachable elements having correlated indicia printed on an exposed side and either on the concealed side of the element or in a portion of the opposing form section in registry with the detachable element. A game or educational piece may be inexpensively provided by correlating different indicia messages corresponding to respective detachable elements, so that in response to an initial query or instruction, a plurality of possible results may be revealed, leading in turn to an outcome revealed by removal of a further detachable element.

Receipts (34a) may be attached to the underlying adhesives (32) by removing the liners (30) and pressing the receipt (34b) onto the exposed adhesives (32). The recording area (20) is located on the reverse side of the mounting area (24). The recording area (20) is a printed surface where the employee can record details pertaining to the receipts attached (34b) to the mounting area (24) of the same sheet.

Abstract: A business form with an integrated label and/or fold-over card is provided. The form includes a form layer having a top surface, a bottom surface, and a periphery, at least one die cut though the top and bottom surfaces within the periphery of the form layer defining at least one portion of the form layer within the die cut, and an adhesively but removably secured patch layer. The patch layer may be secured over the entire die cut and form layer portion to form an integrated label intermediate, or half of the patch layer may be secured over the die cut and form layer portion to form a fold-over card intermediate. In both embodiments the adhesive layer remains on the bottom surface of the patch when the label or card intermediate is removed from the form. By avoiding the use of any release materials, liners, layers and coatings, the remaining form layer is completely recyclable.

Abstract: The blank is made of a sheet of paper of a size suitable for processing through imaging machines, such as ink jet printers and laser printers. For example, the blank may be made 8½ by 11½ inches. The blank includes a front side for receiving printing thereon and at least one removable section in the card. The removable section is formed with a fold line that divides the section into two equal parts for subsequent folding onto each other. Both parts of the removable section are coated with an adhesive on the back side and a polymer sheet, for example, of polyester, is provided over the adhesive on one part of the removable section. A backing sheet, for example, of silicone coated paper is provided over the adhesive on the other part of the removable section. The blank may be provided with a plurality of removable sections so that multiple coupons may be formed from a single blank. In addition, the blank may be printed on both the front side and back side in order to provide information.

The mailing form includes a top ply of the mailing form and a document in the second panel. The mailing form further includes a bottom ply coupled to the top ply and a frame portion of the top ply. The mailing form further includes an adhesive on the outer edges of the front of the bottom ply extending over the second and third panel. The mailing form further includes a first horizontal fold line between the first and second panels and a second horizontal fold line between the second and third panels. When the mailing form is in folded form, the document in the second panel is securely situated between the first and third panel.

An appropriate gift card form can be printed at a time after manufacturing to allow a vendor to personalize the gift card. The gift card form can be kept as a record with the order information.

The informational booklet is attached to at least one of the surfaces of the base card and contains at least one non-adhered edge and at least one adhered edge with the adhered edge being the leading edge for feeding the promotional card into a high speed processing machine that will feed the promotional card into a multi-page article.

Abstract: A business form with laser compatible punch-out card and the methods of making the combination. The card is permanently adhered to a perforated backer sheet, the periphery of which extends beyond the card and is coated with adhesive for application to the bottom surface of a form containing a hole such that the card is disposed within the hole. The card and attached portion of the backer sheet can then be removed from the card-in-form combination by breaking the perforations. The card is die cut from card stock laminated to the backer layer and is prepared for application to the form by stripping a waste matrix of card material from the laminate through the use of patterned release material.

Abstract: A photo album is disclosed, made from a unitary strip of image bearing media having images printed in a pre-determined sequence. A plurality of pages is formed by folding the image media strip between the images into accordion folds and adhering the folds together so that each pair of the images forms two sides of a page. The photo album may have reinforced front and rear photo album covers. A binding holds the pages together.

Abstract: A menu book includes a plurality of pages having transparent sleeves into which menu sheets can be inserted. A flexible, heat-sealable flange extends along a selected edge of each page. The pages are superimposed with the flanges being disposed adjacent each other. The flanges are heat-sealed together to bind the pages into a book. Preferably, the pages and flanges are made of textured vinyl. A particularly desirable method for manufacturing the pages includes the steps of providing a first sheet of transparent heat-sealable material, providing a second sheet of transparent heat-sealable material, superimposing the first and second sheets, forming first and second laterally spaced pages by joining the first and second sheets to each other with heat-sealed seams, the seams of the first and second pages that are adjacent to each other being spaced to define a web therebetween.

In use, the sender of the card dials the telephone number and is prompted to leave a voice message for the addressee of the card. Once the message has been recorded, the sender is provided with an identification code, which is entered in the receiving zone. The message and associated identification code are stored in a central database. Once the addressee receives the card, he or she needs to dial the telephone number indicated on the card to retrieve the sender's message. Upon dialing the indicated telephone number, the addressee will be prompted to enter or provide the identification code. Once this is done, the pre-recorded message, which was left by the sender, is retrieved and played to the addressee.

Abstract: A computer-controlled identifier tag production system. The system employs perforated plastic sheets comprising a plurality of transparent identifier tags. The sheets are designed to be fed through a conventional ink jet or laser printer to print one or more of the tags in accordance with user input. Each sheet is a single layer in thickness and is sufficiently flexible and heat-stable such that it will not deform if passed through a printer multiple times. The printed tags are used in conjunction with a plastic holder and backer plate to form an adhesive-free identifier tag assembly. The backer plate is preferably a rigid metal plate which may be embossed with indicia, such as a corporate logo. The identifier tag assembly may further include a magnet or pin fastener for temporarily affixing the backer plate to an article of clothing.

A preferably transparent an oversheet is attached to the substrate over an insert such as a folded product information sheet. A zone is defined on the oversheet over the insert where the oversheet is nonadhesive. The oversheet can capture an inserted item, such as a folded product information sheet, which is intended to be extracted by a consumer by tearing away a portion of the oversheet or by laterally extracting the insert. The oversheet preferably is substantially clear and the edge strips are perforated. A process is disclosed wherein the oversheet provided on a label roll with a killed adhesive zone, and is applied on a production line using a label applicator and series of rollers.

The form has a well, that is created by removal of the form layer portion from within the die cut. The bottom of the well is the top surface of said adhesive layer and the sides of the well are the edges of said die cut. The adhesive layer has a greater affinity for the patch layer than the form layer portion such that when the form layer portion is removed from the form, the adhesive stays adhered to the patch layer. Articles such as photographs may then be inserted into the well.

Major food and non-food category listings are provided on the shopping list sheet, as well as blank lines for listing individual items, sizes, brands, and the like. Coupon reminders are also located proximate to each blank line. A bottom tacky strip is located across the bottom edge of the shopping list sheet, and two side tacky strips are located at opposing side edges of shopping list sheet. To convert the shopping list sheet into an integrated shopping list and coupon folder, the completed shopping list sheet is peeled from a pad of such sheets and folded along a central fold line, such that the bottom tacky strip seals against itself and the side tacky strips seal against each other. The redemption card includes a plurality of coupons detachably cut into the redemption card, a bar code imprinted onto the redemption card, an integrated circuit chip, and/or a scratch-off foil overlying an offer printed on the redemption card. The promotional mailer preferably includes printed matter in the form of a postage area and addressing area outside the redemption card.
