    Abstract: A method of gathering tissue of a heart valve leaflet 2 may include inserting an elongated catheter assembly 12 to a position adjacent the heart valve leaflet, the catheter assembly including a capture tool 22 and a tissue support member 30. The capture tool 22 and the tissue support member 30 may each be independently moveable between a retracted position and an extended position. Partially retracting the capture tool 22 with the tissue support member 30 in the extended position gathers tissue 15 of the heart valve leaflet 2 between the capture tool and the tissue support member. A clip 55 may then be applied from the catheter assembly 12 to the gathered tissue 15 to hold the tissue substantially in a gathered configuration.

With such biasing, when applied to a tissue, such as with a surgical clip applier, the clip can exert a positive clamping pressure to the tissue along the clamping length to seal the tissue and limit passage of fluids, such as blood, from the tissue.

    Abstract: A long articulating steerable clip applier affixed to a user-operated handle. A surgical jaw assembly is attached to the other end of the clip applier. The clip applier is composed of articulating phalanges that are connected end to end by pivoting links and capable of angulations relative to one another when subjected to a tensile force. Each phalange has opposing s-shaped exterior grooves that form two continuous spiral-shaped channels for holding tension wires once the phalanges are assembled. Multiple tension wires are attached to opposite ends of adjacent phalanges. When each wire is pulled, this tensile force causes the phalanges to pivot at equivalent angles with each other. As each individual phalange pivots by an equivalent angle, the sum of these angles causes the free end of the clip applier to pivot by a large angle or a cascading actuation effect.

    Abstract: A tissue fastening apparatus includes a tissue fastening tool, with fastens a first biological tissue and a second biological tissue so as to bring the tissues into close contact and an applicator for indwelling the tissue fastening tool. The tissue fastening tool is made of an elastic wire rod and is wound in a coil shape. The applicator includes a needle tube accommodated in an inner cavity with the tissue fastening tool stretched, a stylet inserted into the inner cavity of the needle tube nearer to the proximal side than the tissue fastening tool, and a rotating mechanism that rotates the stylet around an axis when the stylet is moved toward a distal end of the needle tube. The tissue fastening tool and the stylet are disengageably engaged with each other and integrally and rotatably connected with each other. The rotating mechanism rotates the stylet in a direction opposite to the winding direction of the tissue fastening tool as seen from the proximal side.

    Abstract: A transcatheter method of gathering tissue of a heart valve leaflet may include inserting an elongated catheter assembly to a position adjacent the heart valve leaflet, the catheter assembly including a capture tool moveable between a retracted position and an extended position, a tissue support, and a clamping member moveable between an open position and a closed position. The capture tool may be partially retracted to gather tissue of the heart valve leaflet between the tissue support and the clamping member. The clamping member may then be moved from the open position toward the closed position so as to clamp a substantial portion of the gathered tissue of the heart valve leaflet between the tissue support and the clamping member. Subsequently, a clip may be applied from the catheter assembly to the clamped tissue so as to hold the clamped tissue substantially in a gathered configuration.

Release of the locking feature may be performed by externally insertable tools which engage the flexible snap fit and release the interlocking features. The triggering system may also include a catch which is released upon deployment of a closure element. The catch can be reset to allow the triggering system to be re-engaged into a pre-deployment condition and position.

At least one cutting member may be radially deployed and rotated to sever the positioned portion of organ from a distal portion of the organ. An anvil may be employed to draw another portion of the organ adjacent to the portion of organ positioned adjacent to the fastener face of the staple cartridge. The staples may then be deployed through the adjacent portions of organ and an annular knife may be advanced through the adjacent organ portions to complete the anastomosis.
