    Abstract: Apparatus is described for administering a substance to a subject. A vial contains the substance and a stopper is disposed within the vial and is slidably coupled to the vial. A first threaded element is (a) rotatable with respect to the vial and (b) substantially immobile proximally with respect to the vial during rotation of the first threaded element. A second threaded element is threadedly coupled to the first threaded element. At least a distal end of the second threaded element is substantially non-rotatable with respect to the vial, and the distal end of the second threaded element defines a coupling portion that couples the second threaded element to the stopper. The first threaded element, by rotating, linearly advances the stopper and at least the distal end of the second threaded element toward a distal end of the vial. Other embodiments are also described.

A deflectable interference member is mounted within the injector housing and, in a resting position, blocks movement of the activation button assembly from the unactuated to the actuated position thereof. The activation button assembly is configured to activate the injector when in the actuated position, and is movable from the unactuated position to the actuated position solely upon deflection of the interference element out of the resting position thereof. The cartridge door, when in the closed position thereof and having the cartridge received therein, deflects the interference element out of the resting position thereof.

Rotation of the outer screw in one direction advances the middle screw relative to the outer screw via the first threaded engagement and advances the inner screw relative to the middle screw via the second threaded engagement to telescopically extend the screw assembly into an expanded configuration. The hub and the needle are axially translatable between retracted and injection positions. A needle shield, having an aperture, is movably connected to the housing between first and second positions. The needle extends through the needle shield aperture in the injection position of the needle and the first position of the needle shield, and the needle is blocked from extending through the needle shield aperture in the second position of the needle shield. When the needle shield is in the second position and the hub and the needle are in the injection position, the needle shield covers the tip of the needle and subsequent movement of the needle shield toward the first position axially translates the hub and the needle back toward the retracted position.
