    Abstract: A method and apparatus for precisely controlling particle movement relative to a phacoemulsification needle tip is provided. The design monitors actual vacuum present and calculates a pulse shape amplitude waveform proportional to the amount of measured vacuum. An increase in vacuum indicates that the handpiece/needle is becoming occluded by a large particle. The present design determines whether additional power is required to bump or move a large particle away from the needle tip. The present design may employ a control loop that senses and continuously monitors vacuum. The design may dynamically vary the amount of ultrasonic energy delivered to the surgical area in response to the observed actual vacuum, and can actively vary the amount of power delivered to the surgical area based on the size of the particle and the resultant vacuum realized.

    Abstract: An intraocular lens is disclosed, with an adjustable optic that changes shape in response to a deforming force exerted by the zonules of the eye. A haptic supports the optic around its equator and couples the optic to the capsular bag of the eye. The haptic may include a cap on the anterior and/or posterior surfaces of the lens. The lens may include a force transfer member, such as a hinge, that couples forces from the haptic to the cap, so that a radial force on the haptic changes the curvature of the cap. The haptic and optic may be refractive index-matched. The cap may be made of the haptic material, which is stiffer than the optic material, and can influence the deformation of the lens during accommodation. A cap on the anterior surface may produce an axial movement of the lens in an anterior direction during accommodation. The cap may also protect the surfaces of the optic during handling and installation.

Each of the arms has a proximal end and a distal end, the arms being joined to one another at the proximal ends. The peripheral portion is joined to the distal ends of the arms. The arms form an angle that depends on a size of the capsular bag into which the size indicator is placed. The sizing gauge has a body having a front surface, along with first and second features disposed along or behind the front surface. The features are configured to correspond to an angle that is within a predetermined range of angles of the arms of the size indicator when the size indicator is placed within a capsular bag.

After the natural crystalline lens has been surgically removed, a flexible ring may be temporarily or permanently inserted into the capsular bag for measuring the circumference of the capsular bag. The ring is generally compressible to fit through a surgical incision, then expands to fill the capsular bag along an equatorial region. The ring has a central component that changes shape as the ring is compressed. The shape change is generally correlated to the size of the capsular bag and may be measured visually or with a camera through the cornea, the measurement being generally independent of corneal magnification.

    Abstract: An intraocular lens for correcting or reducing the astigmatism of a cornea includes a pupil that is spatially divided into discrete zones, with each zone having a particular astigmatism magnitude and astigmatism orientation. In one embodiment, the zones all have the same astigmatism magnitude, which is equal and opposite the cornea astigmatism magnitude to within a particular tolerance, such as 0.25 diopters. In one embodiment, some or all of the zones all have different astigmatism orientations, with the angular separation between astigmatism orientations being on the order of the rotational misalignment tolerance of the lens to the cornea. The visual performance of such a lens deteriorates more slowly with rotational misalignment, when compared to a comparable lens having a uniform astigmatism orientation across its entire pupil, leading to more relaxed tolerances for a surgeon that implants the lens.

    Abstract: Apparatus and methodology for inserting an intraocular lens (IOL) into an eye. The apparatus includes a cartridge for holding an IOL and a handpiece with a body member for receiving the cartridge, an injector rod that is receivable by the body member, and a drive mechanism that longitudinally oscillates the injector rod in the body member when a rotational force is applied about a longitudinal axis in a single direction. In other words, a constant rotation in a single direction of the drive mechanism causes the injector rod to move distally and proximally in discrete increments, with the distal increments being larger than the proximal increments. Accordingly, the injector rod urges an IOL out of the handpiece in a two-steps-forward, one-step-back manner, thereby minimizing or eliminating the chance of the injector rod engaging with the IOL.
