Abstract: Methods, systems, and software for controlling infusion pressure, such as during a medical procedure, using systemic blood pressure are described. Systemic blood pressure, such as brachial arm blood pressure or radial artery blood pressure, may be used to estimate central retinal artery blood pressure to estimate critical closing pressure. Further, the disclosure relates to controlling infusion pressure to prevent an increase in intraocular pressure above the estimated critical closing pressure when such is not desired, and, when such is desired, using systemic blood pressure and infusion pressure to control an intentional increase in intraocular pressure above the estimated critical closing pressure to stop intraocular bleeding.
Abstract: RNA interference is provided for inhibition of tumor necrosis factor ? (TNF?) by silencing TNF? cell surface receptor TNF receptor-1 (TNFR1) mRNA expression, or by silencing TNF? converting enzyme (TACE/ADAM17) mRNA expression. Silencing such TNF? targets, in particular, is useful for treating patients having a TNF?-related condition or at risk of developing a TNF?-related condition, such as ocular angiogenesis, retinal ischemia, and diabetic retinopathy.

Abstract: Image processing for computer-aided eye-surgery includes acquiring a reference image of the eye and enriching said reference image by inserting additional context information which are helpful for a surgeon when performing the eye surgery. The reference image is registered with a real time image of the eye. The context information is overlayed over the real time image of the eye based on a tracking of the eye movement such that the context information is displayed at the same position despite a movement of the eye.
Abstract: An illuminated microsurgical instrument includes a microsurgical instrument having a distal tip and an optical fiber for delivering a beam of light to a surgical site. The optical fiber includes a proximal end for receiving a light beam from a light source, and a distal end proximate to the distal tip of the microsurgical instrument for emitting the light beam. The distal end includes a beveled end face either oriented toward or oriented opposite from the distal tip of the microsurgical instrument.

Abstract: In certain embodiments, a scanning system comprises optical elements and a movement system. The optical elements comprise an optical fiber and a focusing element. The optical fiber transmits a light ray and has a fiber axis that extends to an imaginary fiber axis. The focusing element refracts the light ray and has a focusing element optical axis that is substantially aligned with the imaginary fiber axis. The movement system rotates the focusing element about the focusing element optical axis to scan the light ray. In other embodiments, a scanning system comprises optical elements and a movement system. The optical elements comprise an optical fiber, a focusing element, and a prism. The prism has a prism optical axis and receives the light ray from the focusing element. The movement system rotates the prism about the prism optical axis to scan the light ray.

Abstract: In various embodiments, a handpiece may include an irrigation line coupled to an adjacent line reservoir with a piston configured to move within the adjacent line reservoir to increase an irrigation pressure in the irrigation line near a surgical site. In some embodiments, a sensor may be used to detect an occlusion break in the aspiration line and information from the sensor may result in signaling an actuator to move the piston in response to the occlusion break detection to compensate for a vacuum pressure drop associated with the occlusion break. The handpiece may further include a purge bypass line between the irrigation line and the adjacent line reservoir to allow flow from the irrigation line to travel through the adjacent line reservoir to purge the adjacent line reservoir.
Abstract: A phacoemulsification device has a cutting tip for insertion into an eye and contact with a cataract. The device includes a shaft having a phacoemulsification movement generator connector end, an applicator tip supporting end, and a shaft section extending between the respective ends of the shaft. The shaft section has a convoluted exposed surface comprising longitudinally extending projections and longitudinally extending valleys arranged in sequence about the circumference of the shaft section. The projections are free of any edges as in, for example, an embodiment that includes projections and valleys that have smooth sloped curving walls in cross section and include a plurality of mirror image S-shaped regions extending from a maximum radial extension of a first projection, down to a maximum depth of a first valley and up to a maximum radial extension of a second projection, with the valley positioned between the first and second projections.
Abstract: A surgical system comprises a hand piece, an irrigation conduit in fluid communication with the hand piece to carry fluid toward a surgical site, an aspiration conduit in fluid communication with the hand piece to carry fluid away from the surgical site, a pump interfacing with the aspiration conduit, the pump to create a vacuum pressure in the aspiration conduit to draw fluid through the aspiration conduit, a vacuum relief valve in fluid communication with the aspiration conduit to relieve the vacuum pressure in the aspiration conduit, a pressure sensor to detect a pressure associated with the surgical site, a controller in communication with the vacuum relief valve and the pressure sensor to control the vacuum relief valve to decrease the vacuum pressure in the aspiration conduit when the pressure detected by the pressure sensor is less than a first pressure threshold.
