  • Patent number: 10350418
Abstract: Devices and methods for improving device therapy such as cardiac resynchronization therapy by determining a value for a device parameter are described. An ambulatory medical device (AMD) can include a sensor circuit to sense a physiological signal and generate two or more signal metrics, and detect an event of worsening cardiac condition using the two or more signal metrics. In response to the detection of worsening cardiac condition, the AMD can determine, for a stimulator, a value of at least one stimulation parameter based on temporal responses of two or more signal metrics. The temporal responses include near-term and long-term responses to the stimulation. The AMD can program the stimulator with the determined parameter value, and generate stimulation according to the determined parameter value to stimulate target tissue.

  • Patent number: 10278653
Abstract: Systems, devices, and methods for managing alerts associated with a target physiological event such as a worsening heart failure event are described. A system may detect one or more alert onsets using an onset threshold, and one or more corresponding alert terminations using a reset threshold. Alerts may be issued corresponding to the detected alert onsets and alert terminations. The system may compare the alerts to a specified alert characteristic, and iteratively adjust the onset or reset threshold until the alerts corresponding to the adjusted onset or reset threshold satisfy the specified alert characteristic. The adjusted onset and reset thresholds may be presented to a user or a process for detecting the target physiological event.

  • Patent number: 10271797
Abstract: Systems and methods for detecting a target cardiac condition such as events indicative of worsening heart failure are described. A system may include sensor circuits for sensing physiological signals and a signal processor for generating a predictor trend indicative of temporal change of the physiological signal. The predictor trend may be transformed into a sequence of transformed indices using a codebook that includes a plurality of threshold pairs each including onset and reset thresholds. The codebook may be constructed and updated using physiological data. The system may detect target cardiac condition using the transformed indices.

  • Publication number: 20190117169
Abstract: Systems and methods are described for subject rehospitalization management. In an example, multiple physiologic signals can be obtained from a subject using multiple sensors. In response to a hospitalization event, pre-hospitalization characteristics of the multiple physiologic signals can be identified. Post-hospitalization characteristics of the multiple physiologic signals can be identified, including characteristics that differ from their corresponding pre-hospitalization characteristics. Later subsequent physiologic signals can be further monitored after the hospitalization event, such as using the same multiple sensors, and subsequent physiologic signal characteristics can be identified. In an example, a heart failure diagnostic indication can be determined using information about the pre-hospitalization characteristics, the post-hospitalization characteristics, and the subsequent characteristics.

  • Patent number: 10182768
Abstract: Systems and methods for detecting a present, or predicting a future, target physiologic event such as worsening heart failure (HF) are described. A system can comprise a patient information receiver circuit, at least two categorical risk analyzer circuits, and a categorical fusion circuit. The patient information receiver circuit receives physiologic signals and generates signal trends. The categorical risk analyzer circuit receives a category-specific input selected from the signal trends according to an associative physiologic condition indicative or correlative of the target event. The categorical risk analyzer circuit produces a signal trend metric indicating relative change in signal strength over time. The categorical risk analyzer circuit calculates a categorical risk index that indicates likelihood of the patient developing or presenting the associative physiologic condition.

  • Patent number: 10098584
Abstract: Systems and methods to monitor an improvement of a subject's physiological condition using a goal improvement profile are described. In an example, a physiological condition of a subject can be compared to a goal improvement profile, where the profile includes at least one target condition indicative of an improved physiological condition detectable using a physiological sensor signal, and a target time when the subject is expected to reach the target condition, where the comparison can be used to indicate whether the subject has met the target condition by the target time.

  • Publication number: 20180168242
Abstract: A nursing slip and method of use include a material formed in a conical shape to conform to the shape of a breast. The nursing slip includes a central opening for passage of the nipple through the nursing slip. The central opening surrounds the areola of the breast. A nursing mother locates the nursing slip around the breast and positions it to surround the areola. The nursing slip is concealed under the clothing of the mother and bra. To nurse an infant, the mother exposes the areola and nipple to the infant. The nursing slip remains to cover the breast from observers and provide a level of modesty in public.

  • Publication number: 20180008204
Abstract: Systems and methods for monitoring patients with a chronic disease are described. A patient management system may sense physiological signals from a patient using one or more implantable or other ambulatory sensors, and generate from the physiological signals a chronobiological rhythm indicator (CRI) such as indicating a circadian rhythm. A reference CRI associated with a prior hospital admission event of the patient may be provided to the patient management system, which compares the CRI to the reference CRI and generates a readmission risk score indicating the patient's risk of subsequent hospital readmission due to a worsened condition of the chronic disease. The readmission risk score may be provided to a user or a process, or used to initiate or adjust a therapy delivered to the patient.

  • Patent number: 9717915
Abstract: Pacing parameters may be adjusted to increase the cardiac output of a patient's heart while a patient is awake and/or active and the demand placed on the heart may be greatest, and to decrease or hemodynamic efficiency while a patient is at rest so that the heart itself has time to rest before the next period of higher demand for efficiency begins. This may aid in lessening the strain placed on the heart by making the heart work hard when needed such as when the patient is active, and by permitting the heart to “rest” when the patient is relatively inactive.

  • Publication number: 20170172496
Abstract: Assessing decongestive therapy delivered to a heart failure patient involves use of an implantable sensor configured to sense a physiologic parameter indicative of the patient's diuresis status and a processor coupled to the implantable sensor. The sensor may comprise a thoracic fluid sensor, a heart sounds sensor, a cardiac chamber or arterial pressure sensor, a respiration sensor, or a blood chemistry sensor, for example. The processor is configured to determine if a target level of patient diuresis has been achieved based on a relationship between the sensed physiologic parameter and a threshold developed for the patient, and to produce an output in response to determining that the target level of patient diuresis has been achieved. The processor may be disposed in an implantable housing, in a patient-external housing, or in a network server system.

  • Publication number: 20170156604
Abstract: An apparatus comprises plurality of sensors and a processor. Each sensor provides a sensor signal that includes physiological information and at least one sensor is implantable. The processor includes a physiological change event detection module that detects a physiological change event from a sensor signal and produces an indication of occurrence of one or more detected physiological change events, and a heart failure (HF) detection module. The HF detection module determines, using a first rule, whether the detected physiological change event indicative of a change in HF status of a subject, determines whether to override the first rule HF determination using a second rules, and declares whether the change in HF status occurred according to the first and second rules.
