    Abstract: Methods, systems, and apparatus include computer programs encoded on a computer-readable storage medium, including a method for providing content. The method includes receiving a request for content for a block and determining a variable number of content items to be responsive to the request, including determining an efficiency for each of a number of permutations of allocations of eligible content items. The efficiency is a summation of individual values associated with content sponsors proposed to be included in the block for a given permutation. A price is established to be charged to each content sponsor associated with a permutation having a highest efficiency. The establishing includes, for each position in a determined highest efficiency permutation: determining a total increase of efficiency associated with all other content sponsors when removing a content sponsor associated with a given position. The eligible content items associated with the determined permutation are provided.

    Abstract: The invention relates to a shift control device for an electronic derailleur of a racing bicycle. The control device includes a support body conventionally mounted to handlebars of the bicycle. The control device includes a plurality of electrical switches for selectively controlling a gear change of the bicycle. An upper set of switches operate an electronic motor of a derailleur for controlling a gear change towards a higher or a lower gear ratio, and are located toward an upper portion of the support body. A lower set of switches are located toward a lower portion of the support body, and also operate the electrical motor of the derailleur for controlling the gear change. The upper set of switches and the lower set of switches are each respectively positioned on the control device to minimize interference and/or mis-shifting during various riding positions by the cyclist.

The reputation index is based on a reputation vector which includes a plurality of factors for the Internet resource such as country of domain registration, country of service hosting, country of an internet protocol address block, age of a domain registration, popularity rank, internet protocol address, number of hosts, to-level domain, a plurality of run-time behaviors, JavaScript block count, picture count, immediate redirect and response latency. If the reputation index for the Internet resource is at or above a threshold value established for the LAN, then access to the Internet resource is permitted.

The dilator fits within the sheath and serves to widen a path through the epidural region for the introduction of a stimulation lead. At least a portion of the stimulation lead introducer has an oblong cross-section, allowing passage of stimulation leads such as paddle leads. The stimulation lead introducer may enter the epidural region proximate a spine of a patient via a guidewire. The stimulation lead introducer provides a path through the epidural region of a patient to a target stimulation site. A stimulation lead may travel through the path to reach the target stimulation site where it may provide therapy to the patient.

The lumen is sized to receive both of the first and second therapy elements simultaneously. The first and second therapy elements are separate and not attached to one another such that distal ends of the first and second therapy elements can be positioned to provide therapy in distinct locations of the patient.
