    Abstract: The present invention provides systems and methods for selectively applying electrical energy to a target location within of a patient's body, particularly including tissue in the spine. The present invention applies high frequency (RF) electrical energy to one or more electrode terminals in the presence of electrically conductive fluid to remove, contract or otherwise modify the structure of tissue structures. In one aspect of the invention, a method is provided for treating herniated discs within a patient's spine by applying sufficient electrical energy to the disc tissue to reduce a volume of the disc, thereby relieving pressure on a spinal nerve. In one embodiment, the high frequency voltage is sufficient to ablate a portion of the nucleus pulposis, either the extruded portion outside of the annulus or a portion or all of the pulposis within the annulus.

    Abstract: Bone fasteners consisting of a rivet and an expansion pin for attaching soft tissue and bone plates to bone. The rivet has an enlarged proximal end, a tubular body portion dimensioned to fit snugly within a hole drilled in a bone, and a distal end having expandable legs. The expansion pin is either slidably or rotatably mounted within an axial bore in the rivet. In a first embodiment, the expansion pin has a metallic traction portion and a conical, preferably bioabsorbable, distal end portion releasably affixed thereto. In use, the rivet is inserted into a pre-drilled hole in a bone. When tension is applied to the traction portion of the expansion pin, the conical end portion progressively advances into the bore thereby forcing the legs on the distal end of the rivet to expand outwardly. When the legs are fully expanded, further progress of the conical portion is arrested by a shoulder stop within the axial bore.

    Abstract: A cannulated threaded bone tunnel dilator that accurately advances through bone and dilates a bone tunnel in a gentle, controlled manner, including a distal tapered, threaded tip and a body mounted on the distal end of a shaft. The proximal end of the shaft includes a drive attachment for rotating the dilator so as to threadingly advance the dilator. In use, a guidewire is driven into bone, and then the surgeon advances the dilator along the guidewire until the tip of the dilator engages the bone. The surgeon then rotates the dilator clockwise so as to threadingly advance the dilator through the bone until the dilator reaches a desired depth. After reaching the desired depth, the surgeon removes the dilator by rotating the dilator counterclockwise and retracting the dilator from the then-compacted bone tunnel. The peaks and troughs of the dilator's thread compact the bone so as to create the dilated bone tunnel.

    Abstract: A tissue cutting device includes a catheter with a window at its distal tip for admitting tissue into a catheter compartment. A cylindrical cartridge in the compartment has a cutting edge that supports an electrically conductive cutting element, e.g. a band or wire. The cutting element and adjacent tissue can be heated to a selected temperature by generating an electrical current through the cutting element. The catheter is maneuverable to position its distal end near the tissue to be cut. The catheter incorporates a vacuum feature that urges tissue into the compartment prior to cutting or severing and assists in holding the tissue during the cutting or severing process. Then, the cartridge is manipulated from the catheter's proximal end to move the cutting edge across the window, cutting the tissue. According to alternative embodiments, the cartridge is either rotated or moved axially relative to the catheter and, in either event may be capable of closing the catheter window when the cut is complete.

An elongated neck of the balloon is secured to the tunneling member. The elongated neck permits the tubular member to be withdrawn slightly from the balloon after inflation to facilitate observation.

    Abstract: An expandable stent having end rings with enhanced strength and radiopacity for implantation in a body lumen, such as an artery, is disclosed. The stent consists of a plurality of radially expandable cylindrical elements generally aligned on a common longitudinal stent axis and interconnected by one or more interconnecting members placed so that the stent is flexible in the longitudinal direction. The expandable cylindrical elements have regions which are subject to high stresses and regions subject to low stresses when the cylindrical elements are radially expanded. In order to increase the radiopacity of the stent, the width of the strut in the low stress region of the stent is designed to be wider than the width of the strut in the high stress regions. If a high radiopaque material is utilized and reduced radiopacity is desired, the width of the strut in the low stress region of the stent can be designed to be narrower than the strut width in the high stress regions.

    Abstract: Disclosed is an open cell stent that has adjacent sets of circumferential struts connected by means of highly flexible, undulating, connecting struts. To decrease outward flaring of the circumferential struts when the pre-deployed stent is advanced through highly curved vessels, each unconnected strut of the sets of circumferential struts has a decreased longitudinal length as compared to the longitudinal length of the circumferential struts that are connected by the flexible connecting struts. To decrease the propensity for outward flaring of the end set of circumferential struts, the longitudinal length of the end set of circumferential struts is shorter than the longitudinal length of the interior set of circumferential struts. Also, the attachment point of the flexible connecting struts is off the center of the curved sections of the circumferential struts, thus also decreasing the tendency for outward flaring of the end set of circumferential struts.

    Abstract: An intraluminal grafting system having a balloon catheter assembly, a capsule catheter assembly and capsule jacket assembly is used for deploying a graft having attachment means in the vessel of an animal body. The system contains a proximal capsule means and a distal capsule means, wherein the graft is disposed within the two capsule means and a removable sheath covers the graft and capsule means. The graft is comprised of a tubular member having superior and inferior ends, having attachment means with wall engaging members secured thereto. The balloon catheter is disposed within the capsule catheter to provide relative movement between the balloon catheter and the capsule catheter, whereby the graft can be removed from the capsule means. The balloon is inflated to firmly implant the graft and attachment means within the vessel. The graft and attachment means remain in the vessel after the intraluminal grafting system is withdrawn.

In a second embodiment, a first sheet is formed into a member having a first leg and half of a stem, a second sheet is formed into a second member having a second leg and half of a stem, and the two stem halves are combined to form the bifurcated stent.     Abstract: A vascular prosthesis is constructed from a structure having interconnected, helically oriented channel-porosity to allow oriented ingrowth of connective tissue into a wall of the prosthesis. The prosthesis can have a small internal diameter of 6 mm or less. Several different methods can be used to produce the prosthesis, including a fiber winding and extraction technique, a melt extrusion technique, and a particle and fiber extraction technique using either a layered method or a continuous method. Furthermore, mechanical properties of the prosthesis are matched with mechanical properties of the host vessel, thereby overcoming problems of compliance mismatch.

The sewing ring may be formed of strip of pericardium or other suitable tissue. Material may be captured between the natural tissue sewing ring and a sub-valvular lip formed on the valve to impart a desired size or shape to the sewing ring. In the case of cryopreserved embodiments of the invention (e.g., cryopreserved human homografts) the sewing ring is attached to the valve by way of a natural tissue suture, biological glue or other attachment component that is capable of undergoing cryopreservation without breaking, excessive weakening, or other changes that prevent it from performing its attachment function.
