    Abstract: Systems, methods, and devices are disclosed for optimizing patient-specific stimulation parameters for spinal cord stimulation. A patient-specific anatomical model is developed based on a pre-operative image, and a patient-specific electrical model is developed based on the anatomical model. The inputs to the electric model are chosen, and the model is used to calculate a distribution of electrical potentials within the modeled domain. Models of neural elements are stimulated with the electric potentials and used to determine which elements are directly activated by the stimulus. Information about the models inputs and which neural elements are active is applied to a cost function. Based on the value of the cost function, the inputs to the optimization process may be adjusted. Inputs to the optimization process include lead/electrode array geometry, lead configuration, lead positions, and lead signal characteristics, such as pulse width, amplitude, frequency and polarity.

    Abstract: Implantable medical systems include implantable medical leads that have magnetic orientation-independent magnetically actuated switches that are placed in the conduction path to the electrode of the lead. Thus, regardless of the orientation of a substantial magnetic field like that from an MRI machine to the lead and switch within the lead, the switch opens when in the presence of that substantial magnetic field. The switch may be placed in close proximity to the electrode such that the opening of the switch disconnects the electrode from the majority of the conduction path which thereby produces a high impedance for RF current and reduces the amount of heating that may occur at the electrode when in the presence of substantial levels of RF electromagnetic energy as may occur within an MRI machine.

A PEMF device includes sensors that detect attributes indicating whether the PEMF device is in use. The PEMF device also includes communication devices that connect it with other devices. The data obtained from the sensors may be used to determine a level of compliance in use of the tissue engineering device with a prescribed treatment regimen for the patient. The data is transferred via a paired UE to a remote server. The remote server stores the data in a database and periodically generates compliance reports. The compliance reports are shared with subscribing access devices including the prescribing physician. The UE pairing with the PEMF device maintains a treatment calendar and dynamically modifies reminders based on current treatment status. The treatment regimen may be updated and sent to the PEMF device.

    Abstract: Compliance voltage generation circuitry for a medical device is disclosed. The circuitry in one embodiment comprises a boost converter and a charge pump, either of which is capable of generating an appropriate compliance voltage from the voltage of the battery in the device. A boost signal from compliance voltage monitor-and-adjust logic circuitry is processed with a telemetry enable signal to selectively enable either the charge pump or the boost converter: if the telemetry enable signal is not active, the boost converter is used to generate the compliance voltage; if the telemetry enable signal is active, the charge pump is used. Because the charge pump circuitry does not produce a magnetic field, the charge pump will not interfere with magnetically-coupled telemetry between the implant and an external controller. By contrast, the boost converter is allowed to operate during periods of no telemetry, when magnetic interference is not a concern.

    Abstract: Battery management circuitry for an implantable medical device such as an implantable neurostimulator is described. The circuitry has a T-shape with respect to the battery terminal, with charging circuitry coupled between rectifier circuitry and the battery terminal on one side of the T, and load isolation circuitry coupled between the load and the battery terminal on the other side. The load isolation circuitry can comprise two switches wired in parallel. An undervoltage fault condition opens both switches to isolate the battery terminal from the load to prevent further dissipation of the battery. Other fault conditions will open only one the switches leaving the other closed to allow for reduced power to the load to continue implant operations albeit at safer low-power levels. The battery management circuitry can be fixed in a particular location on an integrated circuit which also includes for example the stimulation circuitry for the electrodes.
