The frames of the prosthetic heart valves can be coupled to one or more shafts of a delivery apparatus via a threaded connection or a non-threaded connection.

Abstract: A prosthetic heart valve including an annular frame and one or more detachable outer skirts and methods associated therewith. The frame has one or more outer skirts releasbly attached to an exterior surface of the frame. In some examples, an outer skirt is attached to the frame via at least one retaining suture formed by stitches looped around a pull suture and struts of the frame. Portions of the outer skirt are captured between the pull suture and the exterior surface of the frame along a suture line. Removal of the pull suture from the suture line enables the outer skirt and frame to separate as the stitches are pulled through openings in the outer skirt. In some examples, outer skirts can be releasably attached to the frame via bioresorbable couplers. Removability of the outer skirts can facilitate improvements in implant and explant procedures of the prosthetic valves.

Abstract: In one representative embodiment, a gripping and pushing device for a medical instrument comprises an elongated main body defining a lumen. The elongated main body comprises one or more deflectable portions that can be pressed radially inwardly toward a shaft of the medical instrument extending through the lumen. One or more elastomeric gripping layers can be secured to the inner surface of a respective deflectable portion at axially spaced apart attachment locations. The one or more gripping layers are axially deformable relative to the one or more deflectable portions between the attachment locations when manual pressure is applied to the one or more deflectable portions to press the one or more gripping layers against the shaft and move the shaft longitudinally into a patient's body.

The elongate shaft assembly includes an anti-migration element configured to shift between a delivery configuration and an open configuration. The anti-migration element is configured to prevent proximal movement of the expandable framework relative to the tubular member in the open configuration.

Abstract: The disclosure provides valve prostheses and methods of installation. One embodiment of the prosthesis has a generally tubular body adapted for placement proximate a mitral annulus. The tubular body has a generally tubular upper portion adapted to substantially reside in the left atrium above the mitral annulus. The generally tubular upper portion has a first circumferential wall that is outwardly biased to urge against cardiac tissue of the left atrium. The tubular body also includes a lower portion extending downwardly from the generally tubular upper portion, the lower portion being configured to substantially reside in the left ventricle below the mitral annulus. The lower portion of this embodiment can be defined by an generally circumferential wall that extends downwardly from the generally tubular upper portion. The generally circumferential wall has a first circumferential end and a second circumferential end, and defines a circumferential extent therebetween.

Abstract: A system includes a stent positionable within a coronary vein. The stent includes a valve and a stent body coupled to the valve and arranged to transition between a collapsed state and an expanded state. The system includes an intravascular reperfusion therapy device with a sensor and a catheter with a lumen shaped to receive the stent. The intravascular reperfusion therapy device delivers reperfusion therapy to a myocardium of a heart. The system includes a processor circuit that receives physiological data from the sensor, determines a progression of the reperfusion therapy, controls a configuration of the valve and/or the expansion state of the stent such that back pressure is controlled. With the stent in the expanded state, the stent body contacts a wall of the coronary vein and the valve obstructs blood flow in a first direction to generate the back pressure in an opposite, second direction.
