    Abstract: A method for forming a lead body includes contacting a proximal section of the lead body having a lumen and a first lead body characteristic to a distal section of the lead body having a lumen and a second lead body characteristic. The proximal and distal sections are contacted such that their lumens are axially aligned. A lap band is disposed about a portion of the proximal section and a portion of the distal section, and is thermally formed to the proximal and distal sections. Axially compressive pressure is applied to the lap band as the lead body is being thermally formed. The pressure applied is sufficient to result in the lead body having an outer diameter in regions proximally and distally adjacent to the lap band that are substantially the same to an outer diameter in a region formed by the lap band.

The distance of the perforated lines from the edge is determined by a minimum white margin value and a maximum image shift error value for a typical small printer. In addition, the image sections are overprinted, i.e., the image printed on each individual sheet contains an overprinted area around the image section to ensure image overlap of neighboring sheets. The lineal is loaded with respect to the head fixtures, with portions of the lineal being folded about the notches to define folded corners. Head fixtures are moved to clamp on the lineal. The cutting structure cuts the lineal generally at each notch to separate each of the folded corners from the lineal, with each corner of the lineal being defined by a pair of ends. The head fixtures are repositioned so as to provide a space between each pair of ends. A heat supplying structure is inserted into the space to heat each pair of ends and then removed. The head fixtures are moved so that each pair of ends contact and join in a thermally welded manner.

A method for processing using this sheet includes the steps of affixing the sheet to a wafer, dicing the wafer with the sheet affixed thereto, peeling the first substrate and first adhesive layer away from the diced wafer by reducing the adhesion of the first adhesive layer and, thereby, dividing the wafer into a plurality of chips, and peeling the second substrate and second adhesive layer away from each of the chips by reducing the adhesion of the second adhesive layer.
