Abstract: A medical record keeping system includes a housing that has a bottom wall and peripheral wall that is attached to and extends upwardly from the housing. A cover is hingedly coupled to an upper edge of the peripheral wall for selectively opening or closing the bottom wall. A panel has a first side, a second side, an upper edge, a lower edge, a first lateral edge and a second lateral edge. A fold line extends from the lower edge to the upper edge and is positioned between the first and second lateral edges. A pair of cavities extends into the first side and extends downward toward the lower edge. Each of a plurality of containers has an open upper end and each s is attached to an inner surface of the peripheral wall so that the upper ends of the container are directed upwardly.

Abstract: A combination book and slideable storage medium holder such as a CD holder received in a cavity of the slideable book cover. The slideable book cover designed to allow the CD holder to slide alternately to enclose or expose the CD that is associated with the book. The slideable book cover further includes a cutout for viewing the CD when the slideable book cover is in the closed position.

Abstract: A kit for securing records comprising a case, a binder attached to the case and a plurality of sections attached to the binder. Each section comprises a class of similar records to be secured and includes a color-coded divider that identifies the section. Each section further comprises at least one record protector, at least one preprinted color-coded label corresponding in color to color-coded divider with the label attached to the record protector and instructions for assembling and maintaining records associated with the section.

Abstract: The present invention is a system for labeling and displaying information on one or more data storage packages. The present invention relates particularly to providing covers with information on the outer surfaces of compact disc, DVD and other product containers. Labels may be attached to stacks of one or more rigid holders in order to display information on the cover, spine or base. Printed graphic cards may be inserted between the rigid plastic holders and covers of clear, flexible cover material. The clear cover material is formed into a clear pocket attached at the opening edge of the rigid cover and base. Alternatively, printed graphic cards are adhered to the outer rigid holders by placing adhesive on the cover and base holders but not on the spines. Also, a clear pocket may be attached to the spine of a stack of rigid holders with a printed graphic insert sliding into the pocket.

Abstract: A protection, authentication, identification device for a collectable object is described. The device comprises a holder formed for assembly with at least one collectable object to provide protection and preservation of said collectable object. The holder is further formed to resist disassembly and separation with the collectable object, once it is assembled. The protection, authentication, identification device also comprises a data storage device which comprises data of at least one stored digital image of the collectable object. The data storage device is nondetachably secured to the protection, authentication, identification device. And, the data storage device is compatible with a standard computer system for display and inspection of the digital image of the collectable object by a user to aid the user in the authentication of the collectable object. In some embodiments, for example, the collectable object is a coin.

Abstract: A data storage device for use in combination with at least one collectable object is described. The data storage device is formed detached from the collectable object. The collectable object is provided with tamper resistant visual markings. And, the data storage device is provided with tamper resistant visual markings associative with the visual markings of the collectable object so as to provide association of the data storage device with the collectable object. The data storage device is compatible with a standard computer system to provide a user with at least one predetermined characteristic of the collectable object with or without an external database.

Abstract: A booklet including a first page having a first height; a second page connected to the first page and having a second height less than the first height; and a spine defining a line around which the second page can be rotated, wherein the first height is such that the booklet can be disposed with respect to a disc case cover having a cover face, a cover tab having a first surface facing the cover face, a first cover edge, and a second cover edge opposite the first cover edge, the distance between the cover edges being substantially equal to the first height, so as to have its first page lie substantially flat between the cover face and the first surface and its second page capable of rotating around the line while remaining substantially flat and without the second page contacting the cover tab.

Abstract: A collectable display panel and data storage device is described. The invention involves at least one display panel that is formed for housing and displaying at least one collectable object, such as a coin. The display panel has associative visual markings for one or more collectable objects. A data storage device has associative visual markings with at least one display panel, and comprises data related to one or more collectable objects that are housed in the display panel. The data storage device is stored in the display panel, and is compatible with a standard computer system. Thus, it is connectable, directly or indirectly to a computer system or equivalent system by wired or wireless means. In one embodiment of the present invention, the collectable object is a coin.

Abstract: A device for packaging a product which can be included with an article, such as an article containing a first cosmetic product. The device includes a sachet containing a second product, in particular a sample cosmetic product, which may be the same as the first or different. In addition, a sheet is folded so as to delimit a compartment in which the sachet is accommodated, with the sheet including information relating to at least one of the first and second products. A packing assembly is also provided, which includes a package having a container therein for a first cosmetic product, with the folded sheet accommodating the sachet of a second cosmetic product also positioned in the package.

Abstract: A collection case combines a trading card with an actual piece of associated memorabilia to enhance its value. The collection case encapsulates the two items together in a protective plastic slab for safe keeping. The process results in a new collectable product of increased value.

Abstract: An expanded content label including independently accessible information fields. In one embodiment, the label includes an overlaminate and an expanded content device having fan-folded panels and first and second information fields. Optionally, the information fields are in different languages. The overlaminate is secured over the panels so that margins extend beyond opposing edges. Each margin is releasably and resealably adhered to a base label and/or an article. The panels fan-fold open when one margin is released to provide a user with a view of the first information field, but not the second information field, and vice versa when the other margin is released. A related method includes: providing a fan-folded device; placing an overlaminate over the device to form margins, and securing the device to a label base, wherein the margins are independently releasable and resealable to fan-fold open the device and selectively access either information field.

Abstract: A foldable device for carrying and displaying at least one flat article is disclosed, including a foldable unitary member adapted for movement between an open and a closed position. The unitary member includes an inside surface, an outside surface, and top, bottom, left and right side edges and further includes a latitudinal fold located between the top side edge and the bottom side edge for separating the unitary member into upper and lower portions and a longitudinal fold located between the left side edge and the right side edge for separating the unitary member into left and right portions. The lower portion of the unitary member is folded onto the upper portion, and the left portion is folded onto the right portion when the unitary member is in the closed position and the unitary member in such a closed position is approximately one quarter of the size of the unitary member in the open position.

Abstract: The present invention relates to a one-part polymer grout composition tintable to any desired color upon request, as well as to a method of tinting the grout composition. Advantageously, the present one-part polymer grout compositions may be tinted using any of the multiplicity of commercially available universal colorants, according to fandecks of color based upon the same, or by visual or computerized color matching. Similarly, since conventional universal colorants may be employed, the equipment utilized to tint and agitate paint in order to provide any desired color may be utilized to tint the present grout compositions. A kit is also provided, comprising the tintable grout composition and instructions for tinting the grout composition according to the method.

Abstract: A folded and sealed cigarette coupon is inserted typically between the outermost face of a cigarette packet and its transparent overwrap. The coupon is fashioned from a flat blank divided by crease lines into a set of panels and extended at either end to create a corresponding set of flaps. The panels are quarto folded to form a substantially rectangular main body of the coupon with the flaps combining to form a grippable closure tab along one edge of the body, which can be torn off to open the coupon. The flaps making up the tab are equal in number to the folded panels, so that the thickness of the coupon remains uniform from edge to edge in both length and breadth.

Abstract: A greeting product and method for conveying a greeting may utilize a banner-shaped greeting structure and a container therefor, the greeting structure having indicia constituting a greeting message, the greeting structure having an extended position in which it is adapted for being displayed, and having a compacted position, and the container being shaped and sized for receiving the greeting structure in its compacted position. The greeting product and the container may have substantially matching shapes when the greeting structure is in the compacted position. The greeting structure may include a plurality of similarly shaped units, linked together. Portions of a greeting message may be placed on respective ones of the units. The units may be polygonal, or at least partially rounded, circular or any other desired shape. The container itself may also have indicia related to the greeting message.

Abstract: A container configured to secure a top section of the container to a bottom section of the container. The container includes a bottom section, including four side panels and a bottom panel, forming an internal cavity therein. The container also includes a top section that is sized in configured to be placed above the bottom section for enclosing the internal cavity of the bottom section. Small openings are included in two opposing side panels of the bottom section so that an elongate length of flexible material, such as a string or ribbon, may be laced through the holes of the opposing side panels and connected above the top section for securing the top section of the container to the bottom section.

Abstract: The present invention is an organizational system, which includes filing pockets and writing surfaces and is designed to be either removably mounted on a surface using magnets or permanently attached to a surface via nails or tacks or otherwise. The invention allows the user to file and identify papers in a compact and readily accessible manner and to make notations and retain information in a readily accessible manner. The major elements consist of a front side that is comprised of a plurality of labeled pockets for the storage of items and may include a separate removable writing surface. The back side may also contain a plurality of secondary pockets for retaining magnets for mounting to a magnetic planar object such as a refrigerator door. The organizational system of the present invention, in a preferred embodiment, has four filling tabbed pockets and comes in a variety of colors. Included in the filings system is a color coordinated dry erase board, tabs, and a dry erase pen.

The assembly comprising the pouch (10), print (19) and backer substrate (20) are laminated in one pass by a laminating machine (30) that applies both heat and pressure.

The system and method use a plurality of containers for storage of current medications, medications needing refills, and medications discontinued by a physician. Furthermore, discontinued medications can be safely retained in case they are later prescribed again by a patient s physician.

Abstract: A generally flat carton blank is arranged for assembly into a carton shape holding multiple cigarette packs. The carton blank has a defined periphery, and a generally flat promotional sheet overlies the carton blank within the outer periphery thereof. A portion of the promotional sheet is adhesively secured to the carton blank. In another embodiment a generally flat promotional sheet overlies the carton blank essentially within the outer periphery thereof. Both carton blank each with its associated promotional sheet may then be assembled and loaded with cigarette packs utilizing standard assembly machinery and standard procedures. Fold lines on the carton blank and the promotional sheet facilitate assembly of the carton blank into a carton shape with the promotional sheet inside the assembled carton.

The album type storage (17, 37, 19, 23) is helpful, discourages disposal of the packaging, which aids the environment, and allows manufacturer advertising and other printed information to persist beyond the sale. Hinges (43, 45) comprise an inverted rectangular U-shape in section in geometry. Windows (5, 7, 12) may be included to allow a view of the components.

The system includes an envelope having a generally flat body portion with an open end, first and second overlying flap portions extending from the open end and having oppositely facing exterior surfaces. The system also includes a shipping container having a main body and at least one cover portion that cooperates with the main body to enclose the articles. The envelope is receivable between the at least one cover portion and the main body of the shipping container and at least one of the first and second flap portions is affixable to an exterior surface of the container.

The pocket may be configured to receive and hold a personal electronic assistant, a cellular telephone (302), or another personal electronic device. The interior pocket includes a window material that allows visual access to the interior (204) of the pocket assembly through the material of the interior pocket (104). The pocket may be constructed so that a personal electronic assistant held inside the pocket assembly is usable through the window without removing the device from the pocket assembly.

In some embodiments, an aperture (214) is included on the hinge spine, such that a portion of the placard can be seen from the end of a folded sleeve assembly. The sleeve member may be generic, while the placard includes specific indicia related to the received disk.

The container elements include means for retaining the container in a closed position such that the chamber formed thereby is relatively insulated. A sealing member may also be provided to cover said opening, which sealing member must be removed to provide access to the opening. Alternatively, the sealing member may include a second opening and access is provided by rotating the sealing member so that the two openings are aligned.

Information medium is also placed within the interior of the package and is held in place by a holder in the base. The information medium may extend to the upper portion of the cover farthest from the base, in which case, the cover may include anti-tipping measures which support the upper edge of the information medium in at least one direction to prevent tipping of the medium.

Abstract: A checkbook cover has an exterior cover, an interior check and register holder affixed adjacent to the cover, and a slot formed through the exterior cover. A writing instrument, preferably with a pocket clip thereon, passes partially through the exterior cover slot to a chamber between exterior cover and interior check and register holder. The pocket clip encompasses at least a part of the exterior cover. The checkbook cover is formed using standard sheet processing techniques, but includes the additional step of severing at a selected strategic internal location to form the slot. Additional force distributors, preferably in the form of circular cut-outs, may be formed at the terminations of the slot. Heat stamped reinforcements are provided in the preferred embodiment about the slot and force distributors, to further strengthen the cover.

Abstract: Novel, quick, and reliable detection methods for determining the authenticity of optical and/or audio disc articles (such as compact discs, digital video discs, CD-ROM discs, and the like) are provided. Such a method basically entails the incorporation of an identifier or marking means within the edges of target optical discs or within any other portion of a target disc such that exposure to non-visible light sources will provide the desired identification upon irradiation or detection of the marking means therein. Such an identifier or marking agent thus may be utilized in conjunction with a transparent holding case whereby the needed light source may be exposed through the transparent walls thereof to permit detection of counterfeit or authentic discs without requiring opening of the case itself. Such a method may also be utilized for discs that are removed from or have not been placed and sealed within such a case as well.

Abstract: A kit for storing computer discs. The kit includes sheets of die-stamped card stock each of which are folded to form a CD or a DVD holder. The kit also includes a special stackable and expandable disc storage unit. Preferably, the user's computer prints labels on the holder identifying the contents of the CD or DVD. The software programming the computer utilizes may be provided on a CD or it can be downloaded from a WEB site. The disc storage unit is made up of a number of disc storage shelves of an elongated male tab and an elongated female receptor wherein the tabs and the receptor mate respectively with receptors and tabs on adjacent shelves to form the stackable and expandable storage unit. Preferably, the kit also includes end support pieces for preventing the disc storage shelves from sliding relative to each other while stacked. The compact disc holder is inserted into a stackable disc storage unit for storage and display.

The two-ply wall has a slot in one of the plies to receive a CD into a CD retaining recess between the plies. A mailer flap connected to one of the walls along a fold line is folded over the two-ply wall to close the open end of the pocket and cover the CD receiving slot for mailing.
