Abstract: Methods and apparatus for constructing a form are disclosed. A form includes a substrate a first line of weakness formed on the substrate to separate a central area and a perimeter matrix. The perimeter matrix at least partially surrounds the central area. A fold line formed on the central area to divide the central area into a first side and a second side. A second line of weakness and a third line of weakness are at least partially formed within the central area. The second and third lines of weakness are coupled to the first line of weakness. The second and third lines of weakness form opposing removable portions that are symmetric about the fold line. When the central area is folded about the fold line and the opposing removable portions are removed, the second and third lines of weakness substantially align to create an opening for suspending the form.

Abstract: A process for the preparation of thermal insulation sheets fashioned from certain unseparated mixtures of post-consumer plastic articles is disclosed. The mixtures of such articles contain polyethylene terephthalate (PET) and polystyrene (PS) and optionally also articles made of high density polyethylene (HDPE). An unseparated mixture of such articles is provided. This mixture is crushed and shredded to form plastic flakes, and these plastic flakes are then homogenized. Homogenization of the flakes can be carried out either by melt-blending them or by further comminution to produce very fine particles of the mixed plastics. The resulting homogenized mixture of plastic types is then compression molded into sheets.

Abstract: A coupon card assembly for attaching to a support structure is disclosed. The assembly includes a hinge partially cut through first and second layers of material and a liner to define a first and second card portion movable about a pivot axis. The first card portion includes a score line disposed along a width and cut through the first layer of material to define a segment of the first layer of material removable from the first card portion to expose one of a first and second layer of adhesive remaining on the first card portion for attaching the first card portion to the support structure. The score line is spaced from the pivot axis with at least a portion of the hinge continuing to include both of the first and second layers of material such that the hinge remains functional after removing the segment.
