    Abstract: Total knee replacement surgery is improved through custom cuts on the distal femur without resorting to expensive computer navigation. The method involves measurement, on plain radiographs prior to surgery, the amount of bone that would be resected on each knee, medially and laterally on the distal femur. In the preferred embodiments, the predetermined distance is in the range of 8-10 mm, more preferably 10 mm, and the resulting distance from the second line to the apex of the lateral condyle is in the range of 6 to 7 mm. A cutting fixture is provided and used to resect the medial condyle at the predetermined distance and the lateral condyle at the measured resulting distance.

Thus, where a relatively larger femoral provisional component is mounted to a femur, changing to a smaller provisional component (i.e., “downsizing”) can be accomplished by recutting only two of five original femoral cuts. Cut slots provided in each provisional implant are sized and positioned to correspond to the posterior bone contacting surface geometry of the next-smallest provisional implant size, so that the cut slots can be used to further resect posterior femoral surfaces to accept the next-smallest implant component size.

The jig includes a handle at a proximal end and a mouth at a distal end. The mouth is engagable about the body part and defines a substantially flat plane. A guide has a channel with an axis substantially perpendicular to the plane and a support attaches the guide to the jig. The guide can be translatable relative to the mouth while maintaining the axis substantially perpendicular to the plane. The method determines an axis relative to a body part using the jig. The method comprises engaging the mouth about the body part and translating the guide relative to the mouth so as to align the axis of the channel with the axis of the body part.
