    Abstract: A system has an outer catheter and an inner aspirating clot retrieval catheter having an expansile distal tip for flow restriction, improved aspiration efficiency, and a large mouth into which a clot or other obstructions can be retrieved. The clot retrieval catheter can have a support tube proximal of the tip. The expansile tip can be a strut framework, and a flexible, low-modulus cover is disposed around at least a portion of the tip strut framework and the proximal support tube. The distal end of the tip can be encapsulated by a low-friction elastomeric lip for atraumatic contact with the walls of a blood vessel. The tip has a collapsed delivery configuration and expands radially into a deployed configuration. The tip strut framework, support tube, and cover can all have characteristics which enhance the deliverability of the clot retrieval catheter to the target.

An actuation cable can be connected to a plurality of connections of the engaging framework of struts to apply an expansion force, by the struts, as the struts are actuated by the actuation cable from the first radially expanded state to the second radially expanded state. In the first radially expanded state, a distal end of the struts is spaced a first axial distance away from the shaft, and wherein in the second radially expanded state, a distal end of the struts is spaced a second axial distance, different than the first axial distance, away from the shaft.

The distal portion of the spine may be formed into a spine hoop as part of the distal hoops of the tip section. Alternatively, the spine may form a loop around the shaft braid. Spines can be made from metal, polymer, or fiber strands. Polymer spines can be formed using a lamination or coextrusion manufacturing method. The support tube can also have a polymer jacket or membrane disposed around at least a portion of the structure.

A series of polymeric outer jackets can coat or encapsulate the struts of the frame, giving variable stiffness and preventing disengagement of the interlocking structure while the catheter is pushed through tortuous anatomy.

The device can have a collapsed delivery configuration, an expanded deployed configuration, and a clot pinching configuration. The openings of the cells align when the device is deployed within the clot, where the radial force from the expanding bodies urges portions of the clot through the openings. The inner and outer bodies can be configured to be selectively translatable relative to each other, so that the portions of the clot in the cell openings can be compressed and gripped when the translation transitions the device to the clot pinching configuration. The translation can be maintained to pinch the clot as it is retrieved from the patient.
