Abstract: A system is disclosed for handling post transactional credit disputes in real-time via a variety of transactional environments and architectures. The system includes one or more workstations linked to a communication channel and one or more servers with at least one having capability of displaying a plurality of pre-existing dispute handling forms. A party in dispute may access the forms, choose an appropriate dispute handling form, provide data in the pre-defined requested fields on the form, and transmit the form over the communication channel to a server for processing. A server accepts a User ID and password from a user, retrieves and displays a set of dispute handling forms, receives input entered on the forms, routes the forms to a second user in dispute, and makes the forms available for viewing to the second user. The program may further index the data entered on the forms using a code-based method.

Abstract: Systems, methods, and computer readable media are described for communicating an adverse event report directly to an end-user receiving party and tracking one or more adverse event reports. An adverse event may be detected manually or automatically by a system and an electronic adverse event reporting form presented to a user with one or more data fields pre-populated with data from a patient's electronic health record (EHR). The user may enter and edit data in the electronic form and may submit the data in the electronic form directly to an end-user receiving party such as the U.S. Food and Drug Administration. One or more receipts may be received from the receiving party and indicia from the receipts recorded to one or more records. The submitted adverse event reports may be tracked, accessed, and interacted with through a user interface and additional information may be provided to a user therein.

Abstract: Methods for efficiently publishing electronic versions of traditionally paper-based government forms and utilizing the electronic forms for web-based transactional services are disclosed. Standard paper-based government forms are automatically converted to XML format, including data fields for entry of information by customers. Multiple electronic forms are accessible to the customer at a single access point, such as a web site. At the web site, the customer selects a form for filing with any of a plurality of government agencies. When presented to the customer, the selected form is presented with some of its data fields being pre-populated according to information retrieved from a database and specific to that customer. The customer completes the form, and a back-end system integrated with the web site handles the completed form, determines the appropriate government agency for filing the form, and transmits it as a transaction to the determined government agency.

Abstract: A system and method of providing dynamic and customizable medical forms is disclosed. In certain specific embodiments, these dynamic and customizable medical forms may be automatically presented to users based on a predefined series of rules which allow multiple users having different roles in the clinical process to collaborate and contribute to a medical examination report, while at the same time maintaining an independent record of what was contributed and by whom it was contributed.

The agent feedback module may include a vendor identification program, an agent feedback settings program, an agent feedback program, an agent reporting program, an agent summary program, and an agent profile program. Abstract: A unique tool, i.e., “VeriWeb”, is employed to automatically discover and systematically explore Web-site execution paths that can be followed by a user in a Web application. Unlike traditional spiders (or crawlers) that are limited to the exploration of static links, VeriWeb can navigate automatically through dynamic components of Web sites, including form submissions and execution of client-side scripts. Whenever examining a new Web page, the system determines all possible actions a user might perform—be it a link, via a button with a JavaScript handler, or via form submission—and can execute them in a systematic way. In order to systematically test different paths in a Web site, VeriWeb addresses two critical problems: systematic Web site exploration and automatically filling out forms.

Abstract: Methods and systems are disclosed for creating and storing an electronic rental contract for a rental vehicle from a rental car company to permit a user to bypass a rental counter of a car rental facility when creating the electronic rental contract. A rental vehicle reservation can be created for a user, and then a rental proposal can be created and displayed based upon the reservation. Upon acceptance of the rental proposal by the user, the electronic rental contract is created. A kiosk may be used to form the electric rental contract.

Abstract: Methods and systems are disclosed for creating online and storing within a server system a completed electronic rental contract for engaging a rental vehicle from a rental car company's fleet to thereby authorize a user to pick up a rental vehicle without the user visiting a rental counter. In an exemplary embodiment, an electronic rental proposal can be communicated for display to a user on a website, the electronic rental proposal being at least partially responsive to previously received data about the user's future rental of a rental vehicle. Upon acceptance of the rental proposal, a completed electronic rental contract can be created, the completed electronic rental contract resulting in the user being authorized to pick up a rental vehicle in accordance with the completed electronic rental contract without the user visiting a rental counter.

Abstract: A method and system for interactive and automated assistance with document (e.g., forms) completion for use in complying with requirements or otherwise performing functions for which automated documents are useful, and a user interface for a code authoring tool to support development, customization, and revision of such documents. The user interface for document completion includes search functions to identify documents needing completion, highlighting of fields of documents based on priority for completion of those fields, and a compliance check feature to confirm proper document completion. The code authoring tool allows a user to customize the format of data limitations for fields in documents, define selection choices presented, which are linked to the fields, create help, warning, or error messages, build calculations, define business rules, and generate code and associated documentation, which may be optionally exported.

Documenting a patient encounter utilizing a template-based charting system (either electronic or paper-based), and the tracking of this document status and patient clinical status throughout the encounter, for purposes of managing multiple patients and multiple patients' documents, as well as improved communication between providers and assistants.

Abstract: In many situations, the objective of data entry is to obtain a complex or structured value. This has traditionally been accomplished by providing a discrete entry field for each element of data entry. By extending the notion of an entry filter, and introducing the notion of a discrete editable element within an entry field, it is possible to apply a structure to that entry field. A management component enables one or more entry filters to be associated in turn with each editable element, and ensures that the entry filters are applied correctly to the appropriate elements.

The search strategies and quick matches that are listed are dynamically updated as the user inputs additional characters in the Find dialog box. At any time, the user can initiate a full text search to populate a search results pane, based upon the characters input, by selecting a Go control or pressing the Enter key. Alternatively, the user can select one of the quick matches or search strategies, to populate the search results pane with corresponding items obtained from the content search data, which are most likely to relate to the information desired by the user. This approach is applicable to finding information in almost any data accessed locally, or over a network.

A predictive logic autonomously completes a web form by determining a requested data type of one or more form objects in the web form. The predictive logic determines a form preference corresponding to an identifier of a selected form object and selects or inserts data of the form preference into the form object. The form preference may be predefined by a user and stored within a form preference database which contains data selections and/or insertions for specific form objects for web forms. The form preference may also specify conditions for triggering events or actions of a web browser. The predictive logic monitors user insertions of a web form and prompts a user to define additional form preferences.

Abstract: The present invention provides a unique system and method that can employ machine learning techniques to automatically fill one or more fields across a diverse array of web forms. In particular, one or more instrumented tools can collect input or entries of form fields. Machine learning can be used to learn what data corresponds to which fields or types of fields. The input can be sent to a central repository where other databases can be aggregated as well. This input can be provided to a machine learning system to learn how to predict the desired outputs. Alternatively or in addition, learning can be performed in part by observing entries and then adapting the autofill component accordingly. Furthermore, a number of features of database fields as well as constraints can be employed to facilitate assignments of database entries to form values—particularly when the web form has never been seen before by the autofill system.

Abstract: A method for populating and distributing an approved health insurance carrier form is provided, as well as a machine-readable storage device programmed to carry out the method. The method involves querying a person eligible for health insurance regarding his or her health history and that of any additional insured parties, receiving the responses electronically, and storing the responses in the form of digital data. The responses are then used to automatically generate one or more populated approved forms for one or more health insurance carriers. Reminders and updates can be electronically transmitted to the eligible person when new information is needed. By requiring the person to complete a single computerized questionnaire, as opposed to numerous paper forms, the method and device greatly streamline the process of obtaining health insurance.

A host computer providing the CS website may access a merchant website selling the requested item, and order the requested item on behalf of the consumer, and charging a transaction fee, listing fee, receiving a rebate and/or offering a rebate for performing such. One embodiment allows a consumer to purchase/order multiple items from multiple websites in a single purchase request to the CS website.

Abstract: An apparatus, system, and method for quantifying, aggregating and bundling green incentives based on green construction enables applying at least a portion of the credits/incentives to financial transactions. Quantification includes estimating and/or monitoring usage of utilities, comparing the usage to a predetermined baseline value, and valuating an effectual energy savings that includes actual savings plus incentive values. Quantification relies on a database, which may include credits, deductions, and other green incentives data that contribute to the effectual savings. The effectual savings may be applied to early repayment of a mortgage loan, improved terms for the loan, investment in securities, and/or other trading. The method standardizes values of green incentives in energy units and/or monetary units. The method aids in qualifying the building projects based on a concrete estimation of the effectual energy savings.

The system of the present invention proposes the abstraction of the knowledge required to fill forms into a hierarchy containing three levels including form, process and domain knowledge. Such an abstraction caters to a variety of users with varying expertise, from the novice form-filling user to the expert. By encapsulating knowledge in this manner, the system of the present invention removes the complexity involved in the process of form-filling. Further, methods are proposed within the present invention to automatically fill and verify forms, used for a variety of end-uses.

A hierarchical data processing engine may perform partial transformations of data files and produce partial rendering files of changes made to data files. By so doing, the hierarchical data processing engine improves a user's editing experience by allowing for quicker updating of the rendered form being edited by the user.
