  • Patent number: 8512326
Abstract: Tissue may be cut and extracted from an interior location in a patient's body using a probe or tool which both effects cutting and causes vaporization of a liquid or other fluid to propel the cut tissue through an extraction lumen of the cutting device. The cutting may be achieved using an electrosurgical electrode assembly, including a first electrode on a cutting member and a second electrode within a cutting probe or tool. Thus, over a first cutting portion, radio frequency current may help cut the tissue and over a second or over transition region, the RF current may initiate vaporization of the fluid or other liquid to propel the tissue from the cutting device.

  • Patent number: 8512363
Abstract: A scalpel guide system for a bladed surgical instrument. A pair of wire guides attach to a scalpel, for controlled travel of the scalpel along the wire. The wire guides of the scalpel guide system are especially useful as an enhancement to the “Seldinger technique,” or any surgical procedure employed for the percutaneous placement of elongated, tubular devices, with the aid of a guidewire or filament wire. The scalpel can be any knifelike surgical device, conventionally including a blade attached to a handle. The scalpel guiding system preferably includes a pair of wire guides, each extending from the scalpel, which can be disposable, as can the blade, independent of the scalpel. The pair of wire guides each have generally open funnel-shapes, with their wider portion facing the gap formed between the two guides. The user places this medal gap on the wire and rotates the scalpel to engage the wire.

Abstract: A surgical scalpel, scalpel instrument and/or scalpel system (collectively, scalpel), is configured for use in a transverse carpal ligament surgical procedure. The scalpel evaluates an incision path with respect to a nerve in the incision path, and performs the incision if appropriate. The scalpel emits a signal through a potential incision path through tissue captured by the scalpel. The scalpel utilizes the emitted signal to determine whether a nerve is in the incision path. The surgical instrument includes a retractable blade. Evaluation includes determining whether a nerve is present before incision and/or evaluating whether the target tissue has been appropriately captured. A warning is provided when a nerve is in the incision path and/or the captured target tissue is inappropriate. The surgical instrument disables extension of the blade when a nerve is in the dissection path and/or the captured target tissue is inappropriate.

Abstract: A surgical instrument for performing water jet surgical operations comprises a nozzle that can be moved via a hydraulic actuator out of a retracted or passive position into a moved-forward or active position. The actuation is accomplished using hydraulic fluid that is supplied to the instrument head via a tube and/or hose. The fluid (e.g., a sodium chloride solution) that is to be injected into the tissue can be used as the hydraulic fluid that is supplied to the nozzle. High-frequency (HF) current can additionally be applied to the nozzle. The HF current can be conducted to the nozzle via a line extending through the fluid conducting element or via the electrolyte present therein.

Abstract: A sheath for use with a scalpel cutting device including a sheath body configured to cover a scalpel blade at a distal end of a scalpel cutting device. The sheath advantageously may include a plurality of slots formed therein. Providing two or more slots provides additional functionality and/or ease of manufacturing. A single sheath configuration including a plurality of slots (e.g., two) may be manufactured so as to be used with scalpels including somewhat different blade configurations (e.g., which may require a slightly different slot location). Such a universal sheath allows a single sheath design to be used with multiple scalpels, easing manufacture and reducing costs. In addition, depending on the particular location of the slots and the configuration of the scalpel blade, the different slots may each intersect the scalpel blade and provide different characteristics for cutting a suture.
